Clinical Study On Duhuo Jisheng Decoction And Medicine Fumigation In The Treatment Of Liver And Kidney Deficiency Knee Osteoarthritis

Objective:Observing the efficacy of Duhuo Jisheng Decoction combined with medicine fumigation in treating liver and kidney deficiency knee osteoarthritis(KOA)and its effects on serum transforming growth factor-β1(TGF-β1)and insulin-like growth factor-1(IGF-1)),to explore the therapeutic effect of Duhuo Jisheng Decoction combined with medicine fumigation and its possible mechanismMethods:A total of 80 patients with KOA who were treated in the orthopedic clinic of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine between March 2019 and December 2019 were selected.Using random number table method,80 patients were randomly divided into comprehensive treatment group,Chinese medicine group,fumigation group and western medicine group,with 20 cases in each group.The comprehensive treatment group was given Duhuo Jisheng Decoction combined with medicine fumigation treatment;the Chinese medicine group was given Duhuo Jisheng Decoction alone;the fumigation group was given medicine fumigation alone;the western medicine group was given oral celecoxib capsules.Each group of 15 consecutive days of treatment was a course of treatment,with 5 days of rest between courses,and 2 consecutive courses of treatment.The visual analogue scale(VAS)score,the western Ontario and McMaster universities osteoarthritis index(WOMAC)score,the change of total effective rate,and the changes of TGF-β1 and IGF-1 content in serum before and after treatment were compared among the three groups,and statistical analysis was performedResult:1.After the treatment,the total effective rates of the four groups were compared,90%for the comprehensive treatment group,60%for the Chinese medicine group,65%for the fumigation group,and 70%for the western medicine group.The difference was statistically significant(P<0.05),and there was no statistically significant difference in the efficacy of the Chinese medicine group,the fumigation group,and the western medicine group(P>0.05).2.After the treatment,the VAS scores of the four groups were significantly lower than before treatment,and the difference was statistically significant(P<0.05).The VAS score of the comprehensive treatment group was lower than that of the Chinese medicine group,fumigation group,and Western medicine group,and the difference was statistically significant(P<0.05),there was no statistically significant difference in the VAS scores of the Chinese medicine group,the fumigation group,and the Western medicine group(P>0.05).3.After the treatment,the scores and total scores of WOMAC in the four groups were significantly lower than before treatment,and the difference was statistically significant(P<0.05);after the treatment,the scores and total scores of the comprehensive treatment group were lower than those of the Chinese medicine group and the fumigation group,Western medicine group,the difference was statistically significant(P<0.05),after the treatment,the Chinese medicine group,fumigation group,Western medicine group WOMAC pain score,daily activity difficulty score and total score were not statistically significant(P>0.05),The WOMAC stiffness score of the Chinese medicine group was lower than that of the Western medicine group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the WOMAC stiffness score between the Chinese medicine group and the fumigation group and between the fumigation group and the western medicine group(P>0.05).4.After the treatment,the levels of TGF-β1 and IGF-1 in the four groups were significantly higher than before treatment,and the difference was statistically significant(P<0.05).After the treatment,the serum levels of TGF-β1 and IGF-1 in the comprehensive treatment group were higher than those in the Chinese medicine group,the fumigation group,and the western medicine group,and the difference was statistically significant(P<0.05).There was no significant difference in the levels of TGF-β1 and IGF-1 in the serum of the Chinese medicine group,the fumigation group and the western medicine group after the treatment(P>0.05)Conclusion:1.Duhuo Jisheng Decoction combined with medicine fumigation has a significant effect on KOA,can effectively improve the clinical symptoms of patients,delay the course of disease,and is a reliable and effective treatment method.2.Duhuo Jisheng Decoction combined with medicine fumigation may achieve the purpose of delaying the development of knee osteoarthritis and improving symptoms by increasing the levels of TGF-β1 and IGF-1 in serum.
