Objective:To assess the risk factors that affect the amount of blood transfusion in obstetric patients,and to provide strategies for transfusion preparation in high-risk obstetric patients.Methods:A retrospective survey of patients undergoing blood transfusion from January 2016 to December 2016 in the obstetrics department of Shengjing Hospital Affiliated to China Medical University was made.The patients were divided into a small transfusion volume group(leukocyte-reduced red blood cells < 5u)and a medium-large transfusion volume group(leukocyte-reduced red blood cells ? 5u),We record the patient's physical condition,obstetric complications,surgical treatment and application of blood products.The univariate analysis was used to analyze the differences in physiological and pathological factors between the small transfusion volume group and medium-large one.Logistic regression analysis the independent risk factor affecting blood transfusion in obstetric patients.Results:451 obstetric blood transfusion patients were included in the statistics(335 in a small transfusion volume group and 116 in another large transfusion volume group).There were statistically significant differences in age,prenatal hemoglobin,primiparous delivery,uterine fibroids,scarred uterus,dangerous placenta previa,complete placenta previa,placenta implantation,andeclampsia-preeclampsia(P<0.05).There were statistical differences in the use of other blood components and treatment measures between the two groups.Multivariate Logistic regression analysis showed that anemia during pregnancy,placental implantation,and dangerous placenta previa were independent risk factors for massive blood transfusion in patients.Conclusion:The basic diseases of obstetrics affecting the blood transfusion volume of obstetric patients during surgery.Anemia during pregnancy,placental implantation and dangerous placenta previa are independent risk factors that affecting the blood transfusion of obstetric patients.Preoperative multi-disciplinary coordination,transfusion risk and transfusion volume assessment are needed to improve rescue efficiency for obstetric patients with massive haemorrhage.
