ObjectivesTo understand and evaluate the exercise level of patients with type 2 diabetes,to provide scientific basis for targeted exercise intervention;To explore the application values of high intensity interval training among patients with type 2 diabetes,and provides theoretical basis and practical guidance for enriching the exercise content and developing personalized exercise care programs of diabetic patients.Methods1.The investigation of the situation on the exercise level: The self-made questionnaire and physical activity rating scale were used to investigate the general situation,exercise level and obstacles of the 206 patients from July to October 2018.2.The application of high intensity interval training in patients with type 2 diabetes: 108 patients with type 2 diabetes were selected as the research object,they were randomly divided into A group(high intensity interval training group),B group(moderate intensity continuous training group)and C group(control group).Three groups received the same treatment and health education,the only difference was exercise intervention in the A group and B group.The A group used the method of alternating 1 min high intensity training(heart rate reached 70 %?90 % of the maximum heart rate and fatigue index reached 16?19 levels)and 1 min rest on the basis of the C group,4/6/8/10 cycles per exerciese(grows longer by the week,maximum of 10),each 10 min?30 min,3 times a week.The B group used the method of continuing 20 min?30 min moderate intensity continuous training(heart rate reached 50?70 % of the maximum heart rate and fatigue index reached 12?15 levels)on the basis of the C group,accumulated 150 min/week.The above three groups were observed for 12 weeks,followed by a comparison of the three groups of patients with fasting plasma glucose(FPG),2h postprandial blood glucose(2hPG),hemoglobinA1c(HbA1c),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),triglyceride(TG),the exercise-induced feeling inventory EFI(energetic stimulation,physical and mental calm,active input,physiological fatigue during the exercise),self-efficacy for exercise scale(SEE)and exercise completion rate etc.The observed indicators were analyzed by t test,single-factor anova analysis and nonparameter testing of the IBM SPSS Statistics 21.0 statistical software.Results1.The situation of the exercise level on type 2 diabetesIn the study analysis of patients,56.34 % of patients exercised for more than 20 min per time;And 52.79 % of patients exercised more than 3 times a week;The level of exercise from large to small accounted for 8.63 %,22.84 % and 68.53 %;Regular exercise only accounted for 38.07 %;The main factors that hinder patients' exercise were lack of perseverance for long periods of time,lack of time and weather environment etc.2.The effects of the high intensity interval training on type 2 diabetesThe difference was not statistically significant in the three groups of patients with the glycolipid metabolism,the scores of EFI and SEE before intervention(P>0.05).(1)After 12 weeks of intervention,the average value of FPG,2hPG,HbA1 c,TC,LDL-C,TG in the A group and the FPG,2hPG,HbA1 c,TC,LDL-C in the B group were better than before intervention;The FPG,2hPG,HbA1 c,TC,LDL-C in the A group and the FPG,2hPG,HbA1 c,TC in the B group were better than the C group;The 2hPG and HbA1 c in the A group were better than B group;The comparative differences in the above indicators were statistically significant(P<0.05).There was no significant difference between other indicators(P>0.05).(2)After the intervention,the scores of the energetic stimulation,physical and mental calm,active input,physiological fatigue during the exercise in the A group and the physical fatigue score in the B group were better than before intervention and the C group;The scores of the energetic stimulation and active input in the A group were better than those in the B group;The comparative differences in the above scores were statistically significant(P<0.05).There was no significant difference between other scores(P>0.05).(3)After the intervention,the scores of the SEE in the A group and B group were better than the before intervention and the C group;The A group had better scores and exercise completion rate than those in the B group;The comparative differences in the above scores were statistically significant(P<0.05).There was no significant difference between other scores(P>0.05).Conclusions1.The prevalence of type 2 diabetes tends to be younger,the level of exercise of patients with type 2 diabetes are generally low,and the regular exercise habits are poor,the main factors that hinder the patient's exercise include the lack of long-term exercise perseverance,lack of large amount of exercise time,weather or environmental reasons,so it is urgent to explore short-term,interesting,convenience and diversity of exercise programs to improve the patient's exercise compliance and exercise level.2.Patients with type 2 diabetes had similar general fatigue in high intensity interval training and moderate intensity continuous training,both of which effectively change the level of blood glucose and lipid,but high intensity interval training is better to improve the level of 2hPG,HbA1 c,positive emotion,SEE and exercise completion rate of patients than moderate intensity continuous training.It plays a more active role in improving the metabolism of glycolipids,positive emotion and the degree of exercise compliance,which has better scientificity and practicality.It's worth spreading.
