Objective:To analyze the clinical effect of different drug treatment methods on residual symptoms after reduction of benign paroxysmal positional vertigo maneuver,and to improve the understanding and treatment of residual symptoms after reduction of BPPV.Methods:From November 2018 to November 2019,166 patients with otolithiasis diagnosed in the otolaryngology clinic of our hospital were selected,among which 99 were famale and 67 were male,with residual symptoms after corresponding manipulative reduction.The patients were 41-70 years old.The control group was randomly divided into four groups:the control group,the betastatin mesylate group(treated with betastatin mesylate tablets orally),the ginkgo biloba leaf group(treated with ginkgo biloba leaves orally),and the combination group(treated with betastatin mesylate tablets and ginkgo biloba leaves orally).At the first week of manipulative reduction treatment,all patients were re-examined to observe the patients' positional ocular shock and vertigo,and to evaluate the efficacy.Four groups of patients were followed up to record the duration of residual symptoms(head drowsiness,walking instability).All of the patients in the first time to see a doctor after manual reduction,and 1 week when review,1 months of follow-up,respectively fill in vertigo degree of disability rating scale(dizziness handicap inventory DHI).The age of the four groups of patients was compared by one-way analysis of variance,the gender,vertigo degree and the difference of the affected semicircular canal of the four groups were compared by chi-square test,and the inter-group difference of the mean duration of residual symptoms of the four groups was compared by rank sum test.Rank sum test was performed on the mean score of the vertigo disability rating scale for four groups of patients in three periods.Results:The total effective rate in the control group was 71.42%,which was lower than 90.00% in the betastatin group,85.37% in the ginkgo biloba leaf group and 95.35% in the combination group.The difference was statistically significant(P<0.05).The total effective rate of the combination group was higher than that of the betastatin mesylate group and the ginkgo biloba leaf group,respectively,and the difference was statistically significant(P<0.05).After 1 month of treatment,the duration of residual symptoms in the control group was higher than that in betastatin mesylate group,ginkgo biloba leaf group and the combination group,respectively,with statistically significant differences(P<0.05).The duration of residual symptoms in the combination group was lower than that in betastatin mesylate group and ginkgo biloba leaf group,respectively,and the difference was statistically significant differences(P<0.05).There was no significant difference in the duration of residual symptoms between the betastatin mesylate group and the ginkgo biloba leaf group(P>0.05).There was no significant difference in the score of vertigo disability rating scale between the four groups at the first visit(P=0.685,P>0.05).At the first week and the first month of the treatment,the DHI score of the control group was higher than that of betastatin mesylate group,the ginkgo biloba leaf group and the combination group,respectively,and the difference was statistically significant differences(P<0.05).The DHI score of the combination group was lower than that of betastatin mesylate group and the ginkgo biloba leaf group,with statistically significant differences(P<0.05).There was no significant difference in the vertigo disability rating scale(DHI)score between the betastatin mesylate group and the ginkgo biloba leaf group(P>0.05).Conclusion:For patients with benign paroxysmal positional vertigo who still have residual symptoms after manipulative reduction,treatment with improved microcirculation drugs can shorten the duration of residual symptoms.In addition,the combined application of betastatin mesylate and ginkgo biloba leaves is more effective in improving residual symptoms,which can improve the clinical treatment effect,shorten the course of disease and improve the quality of life of patients.
