A Study On Iodine Nutrition Status Of Urban And Rural Residents And Its Influencing Factors In Some Areas Of Gansu Province

Objective This study analyzes the nutritional status of iodine and the influencing factors of urinary iodine in urban and rural residents in some areas of Gansu,and provides a certain reference for the scientific iodine intake of urban and rural residents in Gansu.Methods A total of 2885 people from the Gansu region who participated in the National Survey of Thyroid Diseases and Iodine Nutrition Status(Tide)were selected,and 1937 were selected after screening.According to different regions and different genders,the overall iodine nutrition status and urinary i odine distribution of the selected population were analyzed.In order to explor e the influencing factors of urinary iodine,gender,age,salt levels,education l evels,different smoking levels,weight,waist circumference,blood pressure,an d blood lipids were grouped as possible influencing factors,and the difference s in urinary iodine among the groups were compared.Different urinary iodine levels were divided into a low urinary iodine group(UIC<100μg/L),an adeq uate urinary iodine group(UIC=100-199.99μg/L),and a more than adequate g roup(UIC≥200μg/L)as the dependent variables.The factors which have differ ences in urinary iodine among the groups were selected as independent variab les,and the adequate urinary iodine group was used as a control group to en ter logistic regression to analyze the independent influencing factors of differe nt urinary iodine levels.Results(1)The overall urinary iodine level of the selected population was 215.90μg/L(153.00μg/L,292.25μg/L).The rural urinary iodine level of 239.40μg/L(174.30μg/L,320.90μg/L)was significantly higher than the urban urinary iodine level 194.30μg/L(138.90μg/L,264.33μg/L)(P<0.05),and there was no significant statistical difference between male urine iodine level 217.30μg/L(156.35μg/L,289.70μg/L)and female 214.70μg/L(149.30μg/L,296.78μg/L)(P>0.05).(2)The composition ratio of the low urinary iodine group(UIC<100μg/L)in rural areas is lower than that in urban areas(5.76% vs.9.78%,P<0.05),and the composition ratio of the excess group(UIC≥300μg/L)is higher than in urban areas(30.39% vs.15.81%,P<0.05).And the composition ratio of male more than adequate group(UIC=200-299.99μg/L)was higher than that of female(35.16% vs.30.68%,P<0.05).(3)With the increase of age,the UIC level gradually decreased,and the UIC in the group of 31-40,41-50,51-60,and>60 was significantly lower than the group of 18-30(P<0.05).The UIC in the severe salt group is significantly higher than in the mild salt group(P<0.05).With the increase of body weight,the UIC gradually decreased.The UIC of the >72kg group was significantly lower than the 57-63 kg group and ≤56kg group(P<0.05).The UIC of the waist circumference >90cm group was significantly lower than ≤77cm,78-83 cm,84-90 cm group(P<0.05).The UIC of the TG-increased group was significantly lower than that of the TG-normal group(P<0.05),and the UIC level of the LDL-C-increased group was significantly lower than the LDL-C normal group(P<0.05).(4)Taking gender,age,salt levels,education,body weight,waist circumference,TG,LDL-C as independent variables,multivariate logistic regression analysis showed that in rural areas,age is an independent influencing factor of urinary iodine level in the low urinary iodine group(OR=0.978,P<0.05);age and gender were independent influencing factors of urinary iodine levels in the more than adequate urinary iodine group(OR=0.985,P<0.05;OR=1.701,P<0.05).In urban areas,age and body weight were independent influencing factors for urinary iodine levels in the more than adequate urinary iodine group(OR=0.982,P<0.05;OR=0.973,P<0.05).Conclusion(1)The overall iodine nutrition status of residents in some urban areas in Gansu Province is at an adequate level;the overall iodine nutrition level of residents in rural areas is higher than that of urban residents,which is at a more than adequate level.(2)The levels of iodine nutrition in different sexes are different.The overall urinary iodine level of male is not different from that of female,but the composition ratio of more than adequate group in male is higher than that in female.(3)For rural residents,gender and age are independent influencing factors of urinary iodine level.For urban residents,age and body weight are independent influencing factors of urinary iodine level.
