Objective By comparing the differences in cognitive fusion and empirical avoidance scores between patients with obsessive-compulsive disorder and healthy control group,We explored the relationship between obsessive-compulsive score and cognitive fusion and experiential avoidance, and tested the effect of Acceptance and Commitment Therapy(ACT)group counseling on obsessive-compulsive disorder based on the theoretical model of ACT.Methods 100 obsessive-compulsive disorder subjects and 166 healthy control groups were tested by questionnaire to compare the differences of cognitive fusion and empirical avoidance scores between the two groups, and explored the relationship between obsessive-compulsive symptoms and cognitive fusion and empirical avoidance.Subsequently, the study paradigm of pre-test,post-test and follow-up was adopted, and the group psychological counseling of 12 obsessive-compulsive disorder subjects who met the group conditions was carried out ACT,followed up for 2weeks,4 weeks and 8 weeks before intervention, respectively,to test the effect of ACT group counseling on obsessive-compulsive disorder.Results(1)The scores of cognition fusion(t =-11.345,P <0.01)and experiential avoidance(t =-7.995,P <0.01)in OCD group were significantly higher than those in healthy control group.(2)There were significant differences in age variables in the scores of cognitive fusion in the obsessive-compulsive group(F =8.63,P <0.01).(3)The correlation analysis showed that the Y-BOCS total score(r =0.229,P <0.01;r =0.243,P <0.01)and their compulsive thinking(r =0.211,P <0.01;r =0.258,P<0.01),compulsive behavior(r =0.135,P <0.01;r =0.111,P <0.01)were significantly positively correlated with cognitive fusion, empirical avoidance score.Anxiety score(r =0.21,P <0.01;r =0.32,P <0.01),depression score(r =0.276,P <0.01);r =0.415,P <0.01)was also significantly positively correlated with cognitive fusion, empirical avoidance.(4)Regression analysis showed that cognitive fusion(B=0.078)and empirical avoidance(B=0.058)all entered the equation when predicting the total score of the Y-BOCS,their joint interpretation variation was 6.8%;in compulsive thinking, cognitive fusion(B=0.053)and empirical avoidance(B=0.020)all entered the equation, and their joint interpretation variation was 7.1%;in predicting compulsive behavior, only cognitive fusion(B=0.047)entered the equation, and its interpretation variation was 1.8%;in predicting anxiety level, cognitive fusion(B=0.320)and empirical avoidance(B=0.055)all entered the equation, and its joint interpretation variation was 10.4%;in predicting depression level, only empirical avoidance(B=0.531)entered the equation, and its interpretation variation was 17.2%.(5)after intervention was completed, the total score of y-bocs(f =3.37,p =0.035)and the cognitive fusion score(f=7.058,p=0.014)in patients with obsessive-compulsive disorder(f =4.78,p =0.042)were significantly reduced;and the mental health dimension score of sf-36 was significantly improved. Conclusion Cognitive fusion and high level of experiential avoidance in patients with obsessive-compulsive disorder may be important factors for the maintenance ofsymptoms, and group counseling of acceptance and commitment therapy(ACT)can improve the obsessive-compulsive symptoms, psychological flexibility and quality of life of patients with obsessive-compulsive disorder.
