| Obj ective:The clinical and biochemical endocrine hormone levels of 680 patients with Polycystic Ovary Syndrome(PCOS)complicated with insulin resistance were summarized,to explore the distribution of TCM syndrome types,the clinical difference of different TCM syndrome types of polycystic ovary syndrome(PCO S)with insulin resistance(IR),an d the influence of the level of IR in different steady-state models of PCOS with IR on the distribution of TCM syndrome types,sex hormones and various metabolic levels,to analyze the relationship between TCM syndrome types of polycystic ovary syndrome(PCOS)with insulin resistance and the levels of insulin resistance and various endocrine hormones,so as to make TCM syndrome differentiation more obj ective,provide theoretical and practical support for further treatment in the futureMethods:The basic information of 680 patients with PCOS-IR who were admitted to the gynecological clinic of the First Affiliated Hospital of Heilongj iang University of traditional Chinese medicine from October 2014 to December 2019 was collected and analyzed,according to syndrome differentiation and tongue and pulse differentiation,they were classified as spleen deficiency and Phlegm Dampness type(n=165 cases),phlegm and blood stasis type(n=217 cases),kidney deficiency and liver stagnation type(n=196 cases)and kidney deficiency and blood stasis type(n=102 cases),according to HOMA-IR of PCOS-IR,the patients were divided into IR1 group,IR2 group,IR3 Group and IR4 group(n=170 cases),to compare the difference of TCM Syndrome type distribution,clinical characteristics,sex hormone level and glucose and Lipid metabolism among different HOMA-IR patients.Results:1.In this study,680 patients with PCOS-IR were divided into four groups according to syndrome differentiation of TCM:Spleen deficiency and phlegm-dampness type,phlegm-blood stasis type,kidney deficiency and liver stagnation type and kidney deficiency and blood stasis type,the results showed that the distribution of the four syndrome types from high to low was phlegm-blood stasis type(2 17 cases,3 1.9%)>kidney-deficiency and liver-stagnation type(19 6 cases,28.8%)>spleen-deficiency and phlegm-dampness type(16 5 cases,24.2%)>kidney-deficiency and blood-stasis type(10 2 cases,15.0%)2.There were significant differences in body weight,body mass index,waist circumference,hip circumference,waist to hip ratio,incidence of black acanthosis,Lutropin Alfa,Lutropin Alfa/Follitropin Beta,120 Min of Blood Glucose,IR,F asting Insulin,30 Min of Insulin,60 Min of insulin,120 Min of Insulin,180 min of insulin,uric acid and lipid accumulation index among patients with different TCM syndrome types(P<0.05)3.According to HOMA-IR,the patients were divided into IR1,IR2,IR3 and IR4.In IR1 and IR2 groups,the syndrome of deficiency of kidney and stagnation of liver-qi was the main type of Tcm Syndrome,and in IR3 and IR4 groups,the syndrome of phlegm and blood stasis was the main type of TCM Syndrome4.Different HOMA-IR patients in body weight,body mass index,waist circumference,hip circumference,waist to hi p ratio,incidence of black spine skin,LH,LH/FSH,testosterone,sex hormone binding globulin,fasting blood glucose,30 minutes blood glucose,60 minutes blood glucose,120 minutes blood glucose,180 minutes blood glucose,IR,fasting insulin,3 0 minutes insulin,60 minutes insulin,120 minutes insulin,1 80 minutes insulin,glycerin There were statistically significant differences in ester,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol,apolipoprotein B,uric.acid,lipid.accumulationindex,TG/HDL-C,TC/HDL-C,LDL-C/HDL-C(P<0.05).Conclusions:1.The main TCM Syndrome type of PCOS complicated with IR is phlegm and blood stasis.2.There is a correlation between the type of phlegm and blood stasis and the occurrence of IR.3.The patients with spleen deficiency and phlegm dampness type and Phlegm Blood Stasis Type had higher degree of insulin resistance,and showed abdominal obesity,the more serious disorder of sex hormone metabolism,glucose and lipid metabolism. |
