Abstract/Summary:
Objectives To find tumor markers with good sensitivity and accuracy and combine with HPV detection to achieve reliable and early screening of cervical cancer.Methods 1 A total of 3766 cases of gynecological outpatients in Qinhuangdao first hospital and 1760 cases of physical examination population were selected for HPV exa mination,among which 105 cases of cervical cancer group,105 cases of precancerous lesions group and 105 cases of healthy control group were selected.2 Tumor markers were detected by electrochemical luminescence in each group,HPV was detected by pcrreverse dot hybridization,and flow cytometry(FCM)was used by MARS for circulating cell detection.3 SPSS20.0 was used for data statistics,one-way analysis of variance was used for measurement data comparison,χ2 test was used for count data,LSD-t test was used for pice comparison.The difference was statistically significant when P < 0.05.Results 1 The infection rate of HPV in outpatient patients was 26.26%,and that of physical examiners was12.16%.The high infection rate of HPV occurred in two age groups,18-30 years old and 51-60 years old.2 The cervical cancer group had the highest detection rate of all HPV subtypes,followed by the cervical cancer precancerous group and the healthy control group.There were statistically significant differences in the detection rates of HPV16,HPV18 and the overall positive rate between the groups(P < 0.05).There was no significant difference between HPV52 and HPV58(P > 0.05).3 The expression of serum tumor markers was the highest in the cervical cancer group,followed by the precancerous group and the healthy control group,with statistically significant differences(P < 0.05).CA125 in patients with cervical cancer group,SSC,AFP,CEA,A153,CA199 expression is the highest,followed by benign tumor group and healthy control group,the comparison between groups,the difference was statistically significant(P< 0.05)of HPV,CA125,CA153,CA199,specific measures of individual/joint detection,a joint detection under CA199,sensitivity and accuracy,the joint detection is significantly higher than the index separately,the difference was statistically significant(P< 0.05);In terms of the specificity of HPV,CA125,CA153,CA199,SCC,AFP and CEA,the combined test was statistically significant compared with HPV,CA125 and SCC(P< 0.05),while the comparison of CA153,CA199,AFP and CEA with the combined test was not statistically significant.In terms of sensitivity and accuracy,the combined detection was significantly higher than the individual detection.The difference was statistically significant(P< 0.05).4 In the CTC detection group,there were 17 cases of CTC in the 21 cases of cervical cancer,with a detection rate of 80.95%,and 18 cases of precancerous lesions,with a detection rate of 69.23%.Conclusions HPV combined with tumor markers in the detection of cervical cancer in sensitivity and accuracy than the indicators of the detection alone significantly improved,suitable for the early screening of cervical cancer.Different types of HPV infection theoretical basis for the selection of cervical cancer vaccine in this region.Figure11;Table 14;Reference 136...
