Relationship Between Negative Emotional Factors And TCM Syndrome Type Distribution Of Polycystic Ovary Syndrome

?Objective?To analyze the distribution of negative emotions and the distribution law of TCM syndromes in patients with polycystic ovary syndrome(PCOS),and to discuss the correlation between negative emotions of anxiety and depression in different syndromes of PCOS and relevant biochemical indicators preliminarily,aiming at providing certain scientific basis for drawing up treatment and prevention scheme of individualized emotional regulation for patients with PCOS belonging to different TCM syndromes.?Methods?According to the Chinese guidelines for the diagnosis and treatment of polycystic ovary syndrome(2018 Edition),patients met the diagnostic criteria were included and then to conduct syndrome differentiation of TCM and to rate anxiety and depression levels.Biochemical indexes such as gonadal hormone and insulin of patients with PCOS were collected to investigate the correlation between anxiety and depression and related biochemical indexes among different syndromes of PCOS.At last,Excel and SPSS23.0 software were used to analyze the data for drawing conclusions.?Results?1.A total of 126 patients with PCOS were collected in this study,and their ages ranged from 15 to 39 years.In terms of education level,the undergraduate college dominated the proportion(53.97%),in which the lowest was technical secondary school and the highest is master degree candidate.And 39 cases(30.95%)were overweight,90 cases(71.43%)were hairiness and 61 cases(48.41%)suffered acne.107 cases(84.92%)of delayed menstruation was the main disorder of irregular menstruation.There were 71 cases(56.35)with hyperandrogen and 109 cases(86.51%)with polycystic change of ovary.2.Distribution of TCM syndromes: in this study,the frequency of TCM syndromes distribution was successively from high to low: syndrome of kidney deficiency and liver depression(23.80%),syndrome of kidney deficiency(14.29%),syndrome of kidney deficiency and liver depression and phlegm-dampness(12.70%),syndrome of kidney deficiency and liver depression and blood stasis(9.52%),syndrome of liver depression(7.94%),syndrome of liver depression and blood stasis(7.14%),syndrome of kidney deficiency and liver depression and intermingled phlegm and blood stasis(5.56%),syndrome of liver depression and phlegm-dampness(3.97%),syndrome of intermingled phlegm and blood stasis(3.97%),syndrome of kidney deficiency and phlegm-dampness syndrome(3.17%),syndrome of kidney deficiency and blood stasis(2.38%),syndrome of kidney deficiency and intermingled phlegm and blood stasis(1.59%),syndrome of liver depression and intermingled phlegm and blood stasis(1.59%),syndrome of phlegm-dampness(1.59%),and syndrome of blood stasis(0.79%)).3.Inter-group comparison of main TCM syndromes: the top four syndromes of highest frequency in the study was classified into four groups: syndrome of kidney deficiency and liver depression(group ?),syndrome of kidney deficiency(group ?),syndrome of kidney deficiency and liver depression and phlegm-dampness(group ?),syndrome of kidney deficiency and liver depression and blood stasis(group ?).There was no significant difference in age distribution between the four groups(P>0.05).Comparing BMI and score of hairiness,scores of the group ? were higher than that of the group ? and ?,and the difference was statistically significant(P<0.05);There were no statistical differences in the values of LH,FSH,P,E2,T and LH/FSH between the four groups(P>0.05).4.Inter-group comparison of anxiety and depression: in this study,26 patients had PCOS combined with anxiety state(20.63%),and 36 patients had PCOS combined with depression state(28.57%).The score of hairiness and level of androgen in the anxious group were higher than that of the non-anxious group(P<0.05).There were no statistically significant differences in age,BMI,score of hairiness and acne and levels of sex hormone between the depression group and the non-depression group(P>0.05).Anxiety scores of PCOS patients was correlated with depression scores(P<0.01).5.Relationship of TCM syndrome and negative emotions: the detection rate and score of anxiety in PCOS patients belonging to the syndrome of kidney deficiency and liver depression and phlegm-dampness were higher than that of the other three groups(P<0.05).There was no statistically significant difference in the distribution of scores of depression and detection rate of depressive state among the four groups.?Conclusions?1.The main syndromes of PCOS in this study were: syndrome of kidney deficiency and liver depression,syndrome of kidney deficiency,and syndrome of kidney deficiency and liver depression and phlegm-dampness,and syndrome of kidney deficiency and liver depression and blood stasis.2.PCOS patients belonging to the syndrome of kidney deficiency and liver depression and phlegm-dampness had a high incidence of obesity,hairiness and anxiety.3.The scores of SAS and SDS in patients with PCOS were higher than that of domestic conventional models,and anxiety was correlated with depression scores.4.The scores of hairiness and level of androgen in PCOS patients with anxiety state were higher than that of the non-anxiety group.5.The scores of anxiety and detection rate of anxiety state in PCOS patients belonging to the syndrome of kidney deficiency and liver depression and phlegm-dampness were higher than that of the other syndromes.
