Study On The Clinical Effect Of Tonifying Spleen,Tonifying Kidney,Promoting Yang And Reducing Fire With Western Medicine In Treating Oral Lichen Planus(Erosive Type)

Objective:To observe the clinical curative effect of"tonifying spleen,tonifying kidney,promoting yang and reducing fire"and western medicine on oral lichen planus(erosive type).explore and analyze the clinical safety and efficacy of"tonifying spleen,tonifying kidney,promoting yang and reducing fire"in the treatment of erosive oral lichen planus,in order to expand its application scope and provide reference for the latter to use Chinese medicine to treat this disease.Method:From january 1,2018 to june 31,2019,108 patients with oral lichen planus(erosive type)were admitted to the oral mucosal disease clinic of chengdu university of traditional chinese medicine,compliance with oral lichen planus(erosive type)inclusion criteria,and voluntary subjects.The two groups were randomly divided into experimental group("tonifying spleen and tonifying kidney and raising yang and reducing fire"free decoction granule+western medicine therapy)and the control group(placebo+western medicine therapy),each included 54 patients.Before the treatment,the first two groups of jaw adjustment,cleaning teeth,removal of bad repair body and other operations to eliminate local irritation.The experimental group was given"tonifying spleen and tonifying kidney and raising yang and reducing fire"free decoction granule for oral combination of compound Betamethasone injection and the same amount of sterilization injection water lesion local injection,the usage:"tonifying spleen and tonifying kidney and raising yang and reducing fire"free decoction granule,1 package/time,3 times/day,early and late boiling water after dissolution;compound Betamethasone injection and sterilization water disease damage local injection,1/2 weeks.The control group was given oral placebo,which was similar to the traditional Chinese medicine free decoction granule,combined with compound betamethasone injection and equal amount of sterilized water for injection.usage:ibid.The longest time of intervention was 3 months in the trial group and the control group.Visit for 6months.to analyze and compare the efficacy of the two groups,before and after treatment,subjective and objective index score,cellular immune index,ohip-14quality of life table survey score,recurrence and adverse reactions.Results:1.The total effective rate of the two groups was 94.23%in the experimental group and 80.77%in the control group.After the test,P<0.05,there was significant difference in the total curative effect between the two groups,so in terms of total efficiency,the experimental group was better.2.After treatment,the subjective score and objective score of the experimental group and the control group were significantly lower than those of the P<0.05,so the difference was statistically significant.The objective score of the test group after treatment was compared with the objective score of the control group after treatment,P<0.05,the difference was statistically significant;the subjective score of the test group after treatment was compared with the subjective score of the control group after treatment,because P>0.05,the difference was not statistically significant.3.To compare the cellular immune index(CD4~+,CD8~+,CD4~+/CD8~+)before and after drug intervention in the two groups,The CD4~+index,CD8~+index and CD4~+/CD8~+index of the experimental group and the CD4~+index and CD4~+/CD8~+index of the control group were compared before the drug was used,and the difference was statistically significant(P<0.05).The CD4~+indexes of the experimental group and the control group were found to be P<0.05 after the drug intervention treatment,so there was significance;the CD8~+index of the control group was P>0.05 after the use of drugs,so it was meaningless;the CD8~+index and the CD4~+/CD8~+index of the test group were compared with the control group after the drug intervention treatment,and the difference was meaningless because of P>0.05.4.The OHIP-14 quality of life questionnaire scores were compared between the two groups before and after treatment,The scores of OHIP-14 quality of life questionnaire in the experimental group and the control group after intervention were significantly lower than those in the OHIP-14 quality of life questionnaire before intervention,because P<0.05,the difference was statistically significant.The scores of OHIP-14 quality of life questionnaire in the two groups were compared after treatment,because P>0.05,the difference was not statistically significant.5.After the end of the intervention and follow-up for 6 months,the control group(37.5%)was more likely to relapse than the experimental group(13.0%)and the difference was significant(P<0.05).After 3 months of intervention,no significant adverse reactions were observed in the experimental and control groups.It can be seen that tonifying spleen and tonifying kidney and raising yang and reducing fire free decoction granule combined with compound betamethasone injection and the same amount of sterilizing water in the local injection treatment of EOLP patients with high efficacy,high safety and low recurrence rate.Conclusion:"Tonifying spleen and tonifying kidney and raising yang and reducing fire free decoction granule"orally plus compound Betamethasone injection and the same amount of sterilization injection water for local injection therapy in the lesion can adjust the immunity of EOLP patients,improve the discomfort of disease damage,promote disease healing,improve the quality of life of patients,at the same time,it has high efficacy,high safety and small recurrence rate,which is worth popularizing research and application.
