Clinical Observation On Treatment Of Erosive Oral Lichen Planus With Supplying Spleen-stomach,Purging Yin Fire,Elevating Yang Qi Decoction Associating With Triamcinolone Acetonide

Objective: To observe the clinical effect of Supplying spleen-stomach,Purging yin fire,Elevating yang qi Decoction combined with triamcinolone acetonide in the treatment of erosive oral lichen planus(EOLP),in order to explore the therapeutic effect of Supplying spleen-stomach,Purging yin fire,Elevating yang qi Decoction in EOLP,and to provide a basis for the clinical treatment of EOLP with traditional Chinese medicine.Methods: Seventy-two EOLP patients who met the inclusion criteria and had informed consent were selected from the outpatient clinic of mucosal of Stomatology diseases in Affiliated Hospital of Chengdu University of Traditional Chinese Medicine from November 2017 to November 2018.Through random number method,they were divided into treatment group(integrated traditional Chinese and Western medicine treatment)and control group(simple western medicine treatment),36 cases in each group.Oral health education was conducted in both groups,and compound chlorhexidine acetate solution was used for gargling,3 times a day,5-10 ml each time.The control group was injected with triamcinolone acetonide injection and lidocaine hydrochloride injection 1:1,once a week.On the basis of local injection of Western medicine,the treatment group was treated with Supplying spleen-stomach,Purging yin fire,Elevating yang qi Decoction,decocted in water,1 dose a day,3 times a day.Four weeks as a course of treatment,intervention for a course of treatment,re-visit after 3months of withdrawal.Visual analogue scale(VAS),sign score,total effective rate,recurrence rate and adverse reactions were compared between the two groups before and after treatment.Results: 1.After treatment,the VAS score and sign score of treatment group and control group were lower than before treatment,the difference was significant(P <0.01).Both treatment schemes could effectively improve the clinical symptoms and signs of EOLP.2.After treatment,compared with the control group,the VAS score of the treatment group decreased significantly(P < 0.05),indicating that the combination of Supplying spleen-stomach,Purging yin fire,Elevating yang qi Decoction and triamcinolone acetonide local injection is more conducive to alleviating the pain of EOLP patients;there is no significant difference in the score of physical signs between the two groups(P > 0.05).3.The total effective rate was 88.60% in the treatment group and 82.90% in the control group,with no significant difference(P >0.05).4.3 months after the end of treatment,the recurrence rate was 9.70% in the treatment group and 31.00% in the control group.The difference was statistically significant(P < 0.05),indicating that the recurrence rate in the treatment group was lower than that in the control group.5.There were no obvious adverse reactions during the whole experiment,suggesting that there were no side effects in the treatment of EOLP with Supplying spleen-stomach,Purging yin fire,Elevating yang qi Decoction and triamcinolone acetonide.Conclusion: Supplying spleen-stomach,Purging yin fire,Elevating yang qi Decoction combined with triamcinolone acetonide local injection in the treatment of EOLP cansignificantly alleviate pain,with high safety and low recurrence rate.
