Objective In order to understand the epidemic situation of brucellosis in Inner Mongolia,to understand the knowledge of farmers and herdsmen about brucellosis,as well as the livestock breeding methods and personal behavior characteristics,to explore the influencing factors of brucellosis in farmers and herdsmen.To provide ideas and theoretical basis for the decision-making of prevention and control of human brucellosis in Inner Mongolia.Method(1)Descriptive study was used to describe the distribution among three epidemic characteristics of brucellosis in 2014-2018,using chi-square test and linear trend analysis.The Arc Map component in Arc Gis10.2 was used to display the regional distribution.(2)An on-site epidemiological investigation was conducted on the population of farmers and herdsmen in four banners(counties)in Inner Mongolia.Questionnaires were conducted by one person and one book.Blood samples were collected according to the Diagnostic Criteria for Brucellosis(WS269-2007),and laboratory tests and case diagnoses were carried out.Laboratory results were inputted by Excel 2010,and the questionnaire was coded uniformly using Epi Data 3.0 to establish a database,and SPSS24.0 was used for data statistical analysis.The awareness rate of brucellosis was compared usingχ~2 test,and the score of brucellosis knowledge was compared using t test and one-way ANOVA.The chi-square test was used for univariate analysis of livestock breeding methods and behavior habits,and the influencing factors of brucellosis were analyzed by multivariate logistic regression.Result(1)The epidemic situation of human brucellosis in Inner Mongolia has been on the rise in recent years.It has increased by 17.92%and 30.57%in 2017 and 2018 for two consecutive years,respectively.The linear trend has statistical significance(χ~2=13.02,p<0.001).The number and growth rate of reported cases rank first in our country.The age distribution is mainly young adults,with more males than females.The seasonal distribution is high from March to August,and the regional distribution is high in the Middle East.(2)Through the study of 450 farmers and herdsmen,the average age was 47.57±10.60.Male accounted for 68.9%,female accounted for 31.1%,and the education level was mainly primary school and junior middle school,accounting for 76.50%.121 patients were diagnosed with brucellosis,and the infection rate was 26.9%.(3)In the survey of awareness rate of brucellosis among 450 respondents,279(62.0%)were aware of brucellosis,and the overall knowledge score of transmission route and preventive measures of brucellosis was2.57±2.77.The awareness rate of brucellosis was low in women,low in farmers and low in the same level of education,and the difference was statistically significant(p<0.05).The score of brucellosis knowledge was low in women,low in young age,low in farmers and low in education,and the difference was statistically significant(p<0.05).(4)The results of the investigation on the behaviours of the mother and livestock after delivery,the normal production of placenta and abortion products,the visceral ways of the livestock after slaughter,the feeding ways of sheep,and the dietary habits showed that the patients and non-patients were in the process of whether the placenta and abortion products were fed to dogs,eating viscera,selling viscera,grazing in the pasture,centralized feeding,raising in the yard,and using uncooked milk to produce 8 dairy foods.Behavioral habits were different(p<0.05).Multivariate logistic regression analysis showed that immediate boiling of livestock viscera after slaughter(OR=0.376)and non-use of raw milk for dairy production(OR=0.242)were associated with lower risk of brucellosis infection;pasture grazing(OR=2.015)and mixing of human and livestock(OR=2.890)were associated with higher risk of brucellosis infection.Conclusion(1)The overall situation of the brucellosis epidemic in Inner Mongolia Autonomous Region is grim,and the number of reported cases is on the rise,which requires further strengthening prevention and control.(2)Farmers and herdsmen have poor knowledge of brucellosis,low awareness rate and low knowledge score.It is necessary to publicize and educate key groups on the basis of improving the overall awareness rate.(3)It is necessary to educate farmers and herdsmen on correct use of protective equipment,correct handling of abortion products,abandoning wrong breeding methods,and forming good breeding methods and behavior habits.
