Objective To observe the correlation between uric acid(UA),fibrinogen(FIB),and low-density lipoprotein cholesterol(LDL-C)with SYNTAX score in coronary heart disease(CHD)patients with type 2 diabetes mellitus(T2DM)and their clinical value.Methods Randomly collected from January 2018 to January 2019 in tangshan worker hospital heart subject examination,180 cases of patients with CHD admitted to hospital,according to the results of coronary angiography(CAG)and whether diagnosed T2 DM,incorporating clear diagnosis of CHD in patients with T2 DM 127 cases included in the group,will the CAG angiography negative(coronary lesions < 50%)and not diagnose diabetes 53 cases in control group,all the research object accept emergency or undergoing elective coronary angiography examination,according to the result calculated the CAG and SYNTAX score,According to SYNTAX score,127 cases of CHD with T2 DM were divided into low group(27 cases,0-22),middle group(46 cases,23-32)and high group(54 cases,?33).General information of the selected subjects were collected,including age,sex,height,weight,hypertension history,smoking history,diabetes history,etc.After admission,all enrolled patients should complete the detection of basic vital signs(such as axillary body temperature,respiratory rate,blood pressure,heart rate)as soon as possible,as well as auxiliary examinations such as electrocardiogram and echocardiography,and make detailed records.The hematological examination items of all the selected subjects were collected,including liver function,kidney function,blood glucose,blood lipid,coagulation series,ions,cardiac enzymes,blood sedimentation and other laboratory examination indexes.The values of UA,FIB and LDL-C were recorded according to the above clinical data and laboratory examination.A database was established to collect and integrate general clinical data and laboratory data of all patients.Spss22.0 statistical analysis software was used for the test,and P<0.05 was considered statistically significant.Results 1 There was no significant difference in age,sex,body mass index,history of hypertension,smoking history between the CHD group and the control group(P>0.05).The levels of triglyceride,total cholesterol,LDL-C,fasting blood glucose,glycosylated hemoglobin,UA and FIB in the CHD with T2 DM group were all higher than those in the control group,with statistically significant differences(P<0.05).The high-density lipoprotein of CHD combined with T2 DM group was lower than that of the control group,and the difference was statistically significant(P<0.05).2.There were no statistically significant differences in age,sex,BMI,history of hypertension,smoking history,triglyceride,total cholesterol,high-density lipoprotein among the low,middle and high groups(P>0.05),while there were statistically significant differences in fasting blood glucose and glycosylated hemoglobin among the three groups(P<0.05).3.The UA,FIB and LDL-C levels of each group were in the order of > in the high group and > in the low group,and the differences between the groups were statistically significant(P<0.05).4 Correlation analysis: the correlation coefficients of UA,FIB,LDL-C and SYNTAX score in CHD patients with T2 DM were 0.523(P<0.001),0.584(P<0.001)and 0.657(P<0.001),respectively,showing positive correlation.5 Logistic regression analysis: CHD was combined with T2 DM group SYNTAX evaluation branch logistic regression analysis: the analysis results showed that the OR value of UA was 1.005(P<0.05),the OR value of FIB was 3.348(P<0.05),the OR value of LDL-C was 2.892(P<0.05),the OR value of fasting blood glucose was 3.748(P<0.05),and the OR value of glycosylated hemoglobin was 3.535(P<0.05).suggesting the UA,FIB,LDL-C,glycosylated hemoglobin,fasting blood glucose are coronary heart disease with type 2 diabetes risk factor for coronary artery lesion severity.6.ROC curve: the area under the ROC curve of UA,FIB and LDL-C were 0.791,0.819 and 0.818 for diagnosing CHD patients with type 2 diabetes with SYNTAX score ?33 points,suggesting that UA,FIB and LDL-C have certain diagnostic value and reference significance for severe coronary artery lesions(SYNTAX score ?33 points)in CHD patients with type 2 diabetes.Conclusions SYNTAX scores of UA,FIB,LDL-C and CHD patients with T2 DM are positively correlated.UA,FIB and LDL-C are risk factors for coronary artery disease with type 2 diabetes mellitus.Early detection of UA,FIB and LDL-C levels can provide certain clinical predictive value for the assessment of coronary artery disease in patients with CHD complicated with type 2 diabetes.Figure2;Table7;Reference120...
