Objective To investigate the early prediction of laboratory indexes for severe mycoplasma pneumoniae pneumonia(SMPP),and to guide the early recognition of severe mycoplasma pneumoniae pneumonia.Methods Choosing Hebei Province People’s Hospital 113 cases of children with mycoplasma pneumoniae pneumonia between September 2017 and December 2019,according to the severity of the disease were divided into mild group(66 cases),severe group(47 cases),CRP,PCT,LDH,D-dimer,Ferritin,and Immunoglobulin index comparison in the two groups,through SPSS software,using Mann Whitney U test,t test,comparing the difference between the two groups.ROC analyzed the diagnostic value of CRP,Ferritin,Lactate dehydrogenase,D-dimer,Immunoglobulin and combined detection in the diagnosis of severe mycoplasma infection,and calculated the optimal critical value of each index,so as to further provide early warning for severe mycoplasma pneumoniae pneumonia and provide treatment ideas for diagnosis and treatment.Results 1 Among children with mycoplasma pneumoniae pneumonia,the sex difference between mycoplasma pneumoniae in mild cases and mycoplasma pneumoniae in severe cases was not statistically significant.2 There was no significant difference in age between the normal group and the severe group.There was no statistically significant difference between the severe group and the general group in age ≥5 years and age <5 years.3 CRP,LDH,D-dimer,Ferritin and PCT in the MPP severe group were higher than those in the mild group,with statistically significant differences(P<0.05).There was no significant between the mild group and the severe group(P>0.5).4 ROC curve analysis showed that D-dimer,LDH,CRP,Ferritin were above 0.92,0.78,0.70,0.79,respectively,where Ddimer absolute count area under the curve above 0.9,the highest predictive value,the critical value of 1.03 mg / L,the distinction between ordinary MPP and SMPP sensitivity were 83.0%,66.0%,48.9%,66.0%,specificity 84.8%,81.8%,80.3% and 93.9% respectively,the joint detection area under the curve is 0.93,The sensitivity and specificity were 80.9% and 95.5%,which were of high value in the diagnosis of severe mycoplasma pneumoniae pneumonia.5 Immunoglobulin(Ig A,Ig M,Ig G)in mycoplasma pneumoniae pneumonia was higher in the severe group than in the normal group.6 ROC curve analysis show that Ig A,Ig M and Ig G its value all above 0.70,0.78,0.63,respectively,predictive value to a certain extent,the distinction between ordinary MPPS and SMPP sensitivity were 80.9%,72.3%,44.7%,specificity of 57.6%,68.2% and 83.3% respectively,the area under the curve of joint is 0.83,the sensitivity was 85.1%,specificity of 72.7%,can guide clinical early diagnosis of severe pneumonia mycoplasma pneumonia.Conclusions 1 Mycoplasma pneumoniae pneumonia in C-reactive protein,Lactate dehydrogenase,D-dimer,ferritin,PCT index increased,for the early diagnosis of severe mycoplasma pneumonia,has the important clinical reference value,reason should be given early attention,combined detection value higher,can guide the clinician.2 The increase of immunoglobulin(Ig A,Ig M,Ig G)in children with severe mycoplasma pneumoniae pneumonia indicates that there are certain immune response in severe mycoplasma pneumoniae pneumonia,which has certain diagnostic value,combined detection can guide the early recognition of severe mycoplasma pneumoniae pneumonia.Figure [4];Table [7];Reference [141]...
