Objective: The purpose of this study was to study the effect and prognosis of carotid endarterectomy(CEA)in the treatment of carotid stenosis in elderly patients.Methods: A retrospective analysis of patients who underwent carotid endarterectomy at the Department of Neurosurgery from March 2017 to December 2019,collected 35 patients who met the exclusion criteria of this study.Preoperatively,the patient's neurological function was assessed using the NIHSS score,and transcranial Doppler ultrasound(TCD)was performed to understand the blood flow in the stenosed area.Computer tomography or digital subtraction angiography can determine the stenosis and the degree of stenosis.The operation was performed by the same surgeon in the same manner,and the follow-up period was 6 months.Postoperatively,the NIHSS score,TCD,and CTA were used to evaluate the patient's neurological function recovery,blood flow velocity in the stenosed area,and carotid patency,and jointly determine the efficacy of CEA in elderly patients.Results: All the 35 cases were successfully completed.One case had bleeding after the incision,two cases had hyperperfusion syndrome,one had a stroke,and one had a restenosis 6 months after the operation.Followed up for 6 months,the follow-up rate was 100%.In the follow-up patients,the neurological function was significantly improved at 3 and 6 months after operation(P?0.05),and the blood flow velocity was significantly lower at 3 and 6 months after operation(P?0.05).One patient had mild stenosis at 3 months after operation;2 patients had mild stenosis at 6 months after operation,and one patient had moderate stenosis without surgical intervention.The vascular patency rates of patients at 3 and 6 months were 100%,97.14%.Conclusion: Carotid endarterectomy is definitely effective in elderly patients,and it can be used as the first surgical method for elderly patients with carotid stenosis.
