Comparison Of Sonographic Features And Serum Adiponectin Levels Between First Gout And Asymptomatic Hyperuricemia Based On High Frequency Ultrasound

Objective:To compare the ultrasonographic performance of joints and the level of serum adiponectin(APN)in patients with first-onset acute gout(Gout)and asymptomatic hyperuricemia(HUA),and explore the diagnosis value of ultrasound at different stages of gout and the significance of adiponectin in the occurrence and development of gout and hyperuricemia.Methods 50 acute gout patients with first attack,30 patients with asymptomatic HUA and 30 healthy people were selected as Gout group,HUA group and NC group.MTP1 joint,ankle joint,knee joint,MCP1 joint and wrist joint were examined by two ultrasound doctors.The bilateral joints were compared and recorded in detail.At the same time,all the subjects were asked to take 4ml of venous blood in the morning.After the cases were collected completely,the content of APN in serum was measured by ELISA,and the differences between the groups were recorded in detail.Results 1.There was no significant difference between gout group and HUA group in the positive detection rate of MSU(P > 0.05),The positive rate of synovium hyperplasia,synovium blood supply,joint cavity effusion and bone erosion in Gout group was significantly higher than that in HUA group(P < 0.05),The positive detection rate of ultrasonic manifestations in Gout group and HUA group was significantly higher than that in NC group,the difference was statistically significant(P < 0.05).2.There was no significant difference in UA level between Gout group and HUA group(P > 0.05);the UA level of NC group was significantly lower than Gout group and HUA group(P < 0.05).3.The APN level of Gout group was significantly higher than HUA group and NC group,and the difference was statistically significant(P < 0.05);the APN level of NC group was higher than HUAgroup,and the difference was statistically significant(P < 0.05).4.There was a negative correlation between serum APN and UA.Conclusion Ultrasound is of great value in diagnosing subclinical gout and guiding clinical early intervention in gout.Serum APN can affect the inflammation and metabolism processes of gout,and is an important indicator for predicting the onset of gout.
