objective:The aim of the current study was to compare the correlation and difference between horizontal corneal diameter and ciliary sulcus diameter in myopic patients with different anterior chamber depth,and investigate the influence of horizontal corneal diameter and ciliary sulcus diameter on vault after ICL was analyzed.Methods:A retrospective analysis was conducted on 59 eyes of 34 patients with complete data required to receive ICL implantation to correct high myopia from December 2018 to December 2019.42 patients were divided into three groups: shallow anterior chamber group(ACD<2.8mm),middle anterior chamber group(2.8?ACD<3.2mm),and deep anterior chamber group(ACD>3.2mm).All patients with deep anterior chamber underwent ICL implantation.The differences of horizontal corneal diameter,ciliary sulcus diameter,equivalent spherical mirror,anterior chamber depth,corneal curvature,lens thickness,horizontal corneal diameter and ciliary sulcus diameter werecompared.According to the difference between horizontal cornea diameter and ciliary sulcus diameter,patients with deep anterior chamber were divided into two groups,with the difference <0.5mm and the difference of ? 0.5mm,and the difference of ICL postoperative arch height between the two groups was analyzed.Results:In the shallow anterior chamber group there was no significant difference between the horizontal corneal diameter and the ciliary sulcus diameter(P=0.407).Correlation analysis showed that the horizontal corneal diameter was correlated with the ciliary sulcus diameter(r=0.649,P<0.05).In the middle anterior chamber group there was a statistical difference between the horizontal corneal diameter and the ciliary sulcus diameter(P<0.05),and there was a correlation between the horizontal corneal diameter and the ciliary sulcus diameter(r=0.528,P<0.05).In the deep anterior chamber group there was a statistical difference between the horizontal corneal diameter and the ciliary sulcus diameter(P=0.000),and there was no correlation between the horizontal corneal diameter and the ciliary sulcus diameter(r=-0.33,P=0.893).There were statistical differences in horizontal corneal diameter between the three groups(P<0.05),differences between the shallow anterior chamber group and the middle and deep anterior chamber group(P<0.05).The vault of difference<0.5mm group,and difference >0.5mm group was 627.58±206.71?m,809.72±149.50?m respectively,with statistical differences(P=0.005).Conclusion:It is not accurate to estimate the diameter of the ciliary sulcus using the horizontal corneal diameter in ACD?2.8mm patients,and the selection of inappropriate ICL will affect the postoperative vault.For patients with ACD<2.8mm,the horizontal corneal diameter can be used to estimate the ciliary sulcus diameter.The size of ICL may notmatch the actual anterior chamber width of the patient,resulting in the insufficient vault or excessive vault.For shallow anterior chamber patients,the selection of ICL should be considered in combination with the depth of the anterior chamber and the ciliary sulcus diameter...
