| Objective To explore the influence of UCM on the blood gas of newborn umbilical cord in order to improve the quality management of obstetrics,promote the prognosis of newborn and reduce the incidence of neonatal cerebral palsy.Methods From October 2017 to August 2019,full-term newborns delivered by cesarean section in XX Hospital were analyzed retrospectively.According to the method of umbilical cord disconnection,they were divided into two groups:the group of immediate cord clamping and the group of umbilical cord milking,232 cases in the group of immediate cord clamping and 268 cases in the group of umbilical cord milking.There was no significant difference in the age of pregnant women,gestational age and birth weight(P>0.05).The umbilical cord was cut off within 30s-60s after birth and umbilical artery blood was drawn to detect the blood gas analysis.The general situation of jaundice and dyspnea of newborn in one week after birth was statistically analyzed.Results 1.The pH values of the two groups were similar.The BE in the group of immediate cord clamping was higher than that of umbilical cord milking.The PaO2 and Hb in the group of immediate cord clamping were lower than that of the umbilical cord milking.There was no significant difference in pH between the two groups(P>0.05).There was statistical significance in the comparison of BE value(P<0.05),among which,the average rank of the immediate cord clamping group was236.25,the average rank of the umbilical cord milking group was 161.46,and the average rank of the immediate cord clamping group was greater than that of the umbilical cord milking group.The PaO2 comparison between the two groups(P<0.05),which was statistically significant,the average rank of the immediate cord clamping group was 139.75,the average rank of the umbilical cord milking group was 247.25,and the average rank of the immediate cord clamping group was smaller than the umbilical cord milking group.The Hb comparison between the two groups(P<0.05),which was statistically significant.The average rank of the immediate cord clamping group and the umbilical cord milking group was 140.90 and 260.31.Respectively,the average rank of the immediate cord clamping group was lower than the umbilical cord milking group.The incidence of jaundice in the immediate cord clamping group and the umbilical cord milking group was 17.24%and 17.16%.Respectively,χ2 value was 0.0005,P>0.05,there was no significant difference between them.Among them,the rate of jaundice reaching the treatment standard was 80.00%,86.6%,χ2 value was 0.759,P>0.05,there was no significant difference between them.3.The proportion of respiratory distress in the group of immediate cord clamping group and the umbilical cord milking group was 2.44%and 3.44%.Respectively,χ2 value was0.158,there was no difference between the two groups(P>0.05).Conclusion Umbilical cord milking will not lead to the change of pH in cord blood gas,but will lead to the increase of negative value of BE.In the process of clinical delivery operation,the delayed cord clamping can be used as much as possible if the circumstances permit. |
