| Objective:To analyze the distribution of TCM syndromes in patients with coronary heart disease complicated with metabolic syndrome(CHD merged MS group);to explore the difference and relationship between TCM syndromes with coronary heart disease(CHD group);to study the influencing factors of MS components on coronary artery disease;and to study the correlation between TCM syndromes with metabolic syndrome and coronary artery disease.Methods:The patients were selected from January 2019 to January 2020 by cross-sectional study.There were 168 patients with CHD merged MS group,84 patients with CHD,a total of 252 cases.All the selected cases met the inclusion and exclusion criteria.The basic information,coronary angiography,four diagnosis information were collected,and the general data and syndromes data of CHD merged MS group and CHD group were compared.Results:1.General data: of 168 patients with CHD complicated with MS,108 were male and 60 were female,with a median age of 72 years.The history of smoking was 55,or 32.7 per cent;blood pressure ≥130/85 mmHg and/or hypertension,or 148,or 88.1 per cent;pre-diabetes and/or diabetes,124,or 73.8 per cent;2.CHD combined with MS syndrome element distribution: disease location syndrome element concentrated in: heart,lung,kidney,spleen,gallbladder;disease syndrome element concentrated in: blood stasis,phlegm,qi deficiency,qi stagnation,yang deficiency,yin deficiency,dampness,heat,blood deficiency,drink,cold,water stop.3.Severity of CHD combined with MS syndromes :(1)status syndromes rank heart >lung,kidney,spleen > gallbladder(P<0.05).(2)rank blood stasis,phlegm > qi deficiency >yang deficiency > qi stagnation > yin deficiency,heat,dampness,blood deficiency,drink >cold > water stop(P<0.05).4.The data of syndromes in CHD merged MS group and CHD group were compared:there was no significant difference in the severity of syndromes in the same disease location between the two groups(P>0.05),and the rank of syndromes of coronary heart disease combined with metabolic syndrome was higher than that of yin deficiency,blood stasis,phlegm and qi stagnation in CHD group(P<0.05).5.The influencing factors of 5.MS components on coronary artery disease :(1)Gensini score: TG had no effect on coronary Gensini score,abdominal circumference,hypertension,hyperglycemia and HDL-C were independent influencing factors of coronary artery score(P<0.05).Number of coronary artery branches: TG has no effect on the number of coronary artery branches,abdominal circumference,hypertension,hyperglycemia,HDL-C are independent factors of coronary Gensini score(P<0.05).6.The correlation between CHD combined MS syndrome and coronary artery disease :(1)Gensini score: syndrome heart,lung,kidney,blood stasis,phlegm,cold,yang deficiency,drink syndrome grade and Gensini score were positively correlated(P<0.05),heat,qi deficiency syndrome grade and Gensini score were negatively correlated(P<0.05).(2)Coronary artery branch number: disease position syndrome element heart,blood stasis,phlegm,water stop,yang deficiency,qi stagnation,cold syndrome element grade and coronary artery branch number positive correlation(P<0.05),blood deficiency,heat syndrome element grade and coronary artery branch number negative correlation(P<0.05).Conclusion:1.CHD syndromes with MS syndrome is the most widely distributed and has the most serious pathological changes,the lung,spleen,kidney and gallbladder also have different degree of distribution and injury.2.MS components abdominal circumference,hypertension,hyperglycemia,HDL-C are independent factors affecting coronary artery disease.The improvement of HDL-C level,good control of blood pressure,blood sugar and obesity may bring benefits.3.CHD syndromes with MS location,syndromes and coronary artery disease were correlated.The more serious the coronary artery disease may be,the more serious the pathological damage of the disease syndrome,such as blood stasis,phlegm,cold and yang deficiency may bring serious coronary artery disease,suggesting that heart,kidney,blood stasis,phlegm,cold and yang deficiency are the high risk syndromes of coronary artery disease. |
