Background: To systematically review the association between TNF-?-308G/A,-238G/A,-863C/A,-1031T/C and-857C/T gene polymorphism and susceptibility to periodontitis which diagnosised in the clinical examination by a meta-analysis.Methods: Databases of CNKI(Chinese Journal Full-text Database)and the three English databases Pub Med,Web of Science,Embase were electronically searched by widely used computer,and manually searched the references of the included documents,collected separately TNF-?-308G/A,-238G/A,-863C/A,-1031T/C and-857C/T gene polymorphism and susceptibility to periodontitis which diagnosised in the clinical examination.Set the search time from the establishment of the database to March 2020.Two researchers were set up to conduct quality control on the sample inclusion and exclusion criteria,and independently extract relevant sample information.The Newcastle-Ottawa Scale(NOS)was used as the quantitative standard for methodological quality evaluation of the documents that met the standards.In this retrospective analysis study,Stata12.0 software was used.The odds ratio(OR)and 95% confidence interval(95% CI)were used to combine the effect sizes.The publication bias was detected using Begg's test and Egger's regression test.Result: A total of 52 case-control studies were included in this analysis,5519 cases were the case group,and 7260 cases were the control group.Meta analysis results showed: 1.TNF-?-308G/A gene polymorphism had statistical correlation with chronic periodontitis(CP)susceptibility,in the general population(GG vs.AA:OR = 0.514,95%CI:0.395-0.668,P<0.001;GG+GA vs.AA:OR = 0.625,95%CI:0.488-0.800,P<0.001),in the Asians(GG vs.AA:OR = 0.353,95%CI:0.241-0.515,P <0.001;GG+GA vs.AA:OR = 0.480,95%CI:0.338-0.684,P <0.001).After a sensitivity analysis,it was found that there were two genotypes in the Asians that were statistically correlated with CP susceptibility(G vs.A:OR =0.587,95%CI:0.445-0.774,P<0.001;GG vs.GA+AA:OR =0.466,95%CI:0.361-0.602,P <0.001).2.TNF-?-308G/A gene polymorphism was statistically correlated with aggressive periodontitis(AP)susceptibility,in the general population(G vs.A:OR = 0.758,95%CI:0.655-0.876,P<0.001;GG vs.AA:OR = 0.416,95 % CI : 0.262-0.662,P<0.001;GG+GA vs.AA : OR = 0.478,95 % CI : 0.311-0.736,P= 0.001;GG vs.GA+AA : OR = 0.764,95%CI:0.632-0.923,P= 0.005),in the Asians(G vs.A:OR = 0.651,95%CI:0.526-0.806,P<0.001;GG vs.AA:OR = 0.306,95%CI:0.169-0.551,P<0.001;GG+GA vs.AA:OR = 0.384,95%CI:0.227-0.650,P<0.001;GG vs.GA+AA : OR = 0.677,95 % CI :0.523-0.876,P=0.003).3.TNF-?-238G/A gene polymorphism has statistical correlation with CP susceptibility in the general population(GG+GA vs.AA:OR = 1.813,95%CI:1.458-2.254,P<0.001).4.TNF-?-1031T/C gene polymorphism has statistical correlation with CP susceptibility in the Asians(TT vs.CC : OR = 0.557,95 % CI :0.388-0.798,P= 0.001).Conclusion: 1.The GG genotype and GG+GA in the dominant model(GG+GA vs.AA)of TNF-?-308G/A might be the protective factors for CP in the general population and the Asians,the G allele might be the protective factor for CP in the Asians;2.The G allele,GG genotype and GG+GA in the dominant model(GG+GA vs.AA)of TNF-?-308G/A might be the protective factors for AP in the general population and the Asians;3.The GG+GA in the dominant model(GG+GA vs.AA)of TNF-?-238G/A might be the susceptible factor for CP in the general population;4.The TT genotype of TNF-?-1031T/C might be the protective factor for CP in the Asians.
