ObjectiveTo study the related factors of heart failure in patients with acute ST-segment elevation myocardial infarction after reperfusion therapy.MethodsA total of 296 patients with STEMI who were hospitalized in the intensive care unit of the Department of Cardiology in this hospital from February 2018 to April 2019 were selected and divided into 106 patients in the heart failure group and 190 patients in the non-heart failure group according to whether heart failure occurred during hospitalization.According to the ejection fraction,106 patients with heart failure were divided into two groups, and the laboratory test indicators, echocardiographic test indicators and coronary artery disease indicators of the two groups of patients were compared after admission.According to the patient's past history, biochemical indicators, coronary artery disease Situation, analyze the related factors of heart failure and type of heartfailure in STEMI patients.ResultsThe age,troponin,symptom-onset-to-balloon,cholesterol,high-sensitivity C-reactive protein, and creatinine values in the heart failure group were higher than those in the non-heart failure group;the heart failure group diastolic blood pressure, albumin, and left ventricular ejection fraction were lower than those in non-heart failure group;Patients with acute extensive anterior myocardial infarction and multiple coronary artery stenosis and target vessels with anterior descending branch lesions are more likely to have heart failure. The probability of heart failure combined with ventricular arrhythmia is significantly increased.Age,SOTB time, hypersensitive C-reactive protein, and ventricular arrhythmia are the risk factors for STEMI patients with heart failure.A combination of age,SOTB time, hypersensitive C-reactive protein, and ventricular arrhythmia was used to predict the ROC curve of heart failure. The AUC area was 0.831,the sensitivity was 77.4%,and the specificity was 71.4%.ConclusionThe occurrence of heart failure in STEMI patients is related to the patient's age, total cholesterol,low-density lipoprotein cholesterol, hypersensitive C-reactive protein,SOTB time, number and location of coronary artery disease, left ventricular end systolic diameter and left ventricular ejection fraction, etc.Among them, age,SOTB time, whiteblood cell count, ventricular arrhythmia, and target vessels as the anterior descending branch of acute extensive anterior myocardial infarction are important risk factors.
