Background:Acute myocardial infarction is a common cause of acute left heart failure, and the risk factors for acute left heart failure in non-ST-segment elevation myocardial infarction(NSTEMI)are not clear. Objectives:To investigate the risk factors for acute left heart failure in NSTEMI patients without heart failure at admission. Methods:A retrospective study of 334 patients with NSTEMI without heart failure at admission between February 2016 and December 2018 in Hebei General Hospital was performed.According to whether the patients had acute left heart failure in hospital, they were divided into the non-acute left heart failure group(286 cases)and the acute left heart failure group(48 cases). The baseline clinical characteristics of the two groups were compared.Univariate and multivariate Logistic regression analysis were used to analyze the risk factors for acute left heart failure. Results:Age,heart rate at admission, history of chronic renal failure, history of hypertension,PCI,E/A<1.2,EF,risk stratification, creatinine, myoglobin, glomerular filtration rate, erythrocyte count, hemoglobin and hematocrit were significantly different(P<0.05)in two groups.Binary multivariate Logistic analysis showed that history of hypertension, heart rate,EF,E/A<1.2,glomerular filtration rate(e GFR)were the independent risk factors for acute left heart failure. Conclusion:For patients with NSTEMI without heart failure at admission, history of hypertension, heart rate,EF,E/A<1.2,e GFR may be risk factors for acute left heart failure.
