ObjectiveTo investigate the clinical efficacy and safety of Holmium Laser Enucleation of the prostate(HoLEP)and bipolar transurethral plasma kinetic prostatectomy(TUPKP)in patients with various volumes of benign prostatic hyperplasia.MethodsA retrospective analysis was performed in 239 patients with BPH in our hospital from January 2017 to January 2019,including 147 patients underwent HoLEP(HoLEP group)and 92 patients underwent TUPKP(TUPKP group).According to the volumes of the prostate,the above two groups was divided into three subgroups(group1 < 30ml;30?group2?80ml;group3 > 80ml).The basic information,perioperative date and postoperative follow-up parameters was compared between corresponding subgroups.ResultsCompared with the TUPKP subgroups,the HoLEP subgroups showed significant difference in enucleated tissue weight,enucleated tissue ratio(P<0.05),had less blood loss,less bladder irrigation time,less indwelling catheter time,less postoperative hospital time(P<0.01),but there was no significant difference in operation time,the incidence of postoperative stress urinary incontinence in HoLEP group was significantly higher than that in TUPKP group(P<0.05),there was no significant difference between transfusion rate,urethral stricture rate and bladder neck contracture rate(P>0.05),there was no statistical difference between the subgroups of postoperative follow-up IPSS,QoL score,Qmax,Urination volume,urination time and PVR(P > 0.05),Six mouth after operation,every subgroup had significantly decreased in IPSS and QoL score(P<0.01).ConclusionHoLEP and TUPKP were safe and effective in the treatment of BPH of various sizes;HoLEP has less bleeding and more tissue removal than TUPKP,which is more conducive to rapid recovery of patients and reduced hospital stay. |
