Meta-analysis Of Efficacy And Safety Of Microwave Ablation For Hepatic Hemangioma

Objective:To evaluate the efficacy and safety of microwave ablation in the treatment of hepatic hemangioma.Methods:In the study,PubMed,EMBASE,Cochrane Library,CNKI,Wan Fang Database and CBM were searched and included in the comparative experiment between microwave ablation and laparoscopic hepatectomy for the treatment of hepatic hemangioma.The retrieval time limit is from the establishment of the database to December 2019.According to the inclusion and exclusion criteria,two researchers screened literature,extracted data and evaluated the quality of the included literature.Review Manager 5.3 and Stata 12.0 were used for meta analysis.Results:Through retrieval,9 literatures,4 non randomized controlled trials and 5 randomized controlled trials were included.A total of 1108 patients were included,including 594 in the microwave ablation group and 514 in the surgical resection group.Meta analysis showed that compared with laparoscopic hepatectomy group,microwave ablation group had hospitalization time(SMD =-0.19,95% CI:-0.34 ~-0.04,P = 0.01),operation time(SMD =-2.50,95% CI:-2.96 ~-2.03,P < 0.00001),intraoperative bleeding volume(SMD =-3.18,95% CI:-4.02 ~-2.33,P < 0.00001),postoperative VAS score(12h: SMD =-1.34,95% CI:-1.51-1.17,P < 0.00001;24h: SMD =-0.89,95% CI:-1.05-0.73,P < 0.00001;48h: SMD =-0.77,95% CI:-1.02-0.52,P < 0.00001).0.06 ~ 0.48,P = 0.001).Conclusion:Compared with laparoscopic hepatectomy for the treatment of liver hemangioma,microwave ablation for the treatment of liver hemangioma has the advantages of short hospital stay,short operation time,less intraoperative bleeding and less postoperative pain,but the postoperative liver function recovery and cure rate are inferior to laparoscopic hepatectomy.
