Objective:A comparative study of the clinical efficacy of thoracoscopy-assisted rib internal fixation and traditional rib open reduction and internal fixation in the treatment of traumatic multiple rib fractures.Methods:The clinical data of 80 patients with traumatic multiple rib fractures treated in our department from January 2017 to December 2018 were retrospectively analyzed.They were divided into thoracoscopic group(n=42)and traditional surgery group(n=38)according to different surgical methods,the thoracoscopy group underwent thoracoscopy assisted rib internal fixation,and the traditional surgery group underwent open rib fracture incision and reduction and internal fixation.The operation time,intraoperative blood loss,surgical incision length,and pain Scoring on the first day after surgery,drainage tube placement time,postoperative out-of-bed activity time,hospitalization time,hospitalization cost,postoperative complications,stress response indicators,and long-term efficacy evaluation at 6 months post operatively were compared between the two groups.This study was performed using SPSS22.0 statistical software.Data collation and analysis.Count data is expressed as a rate(%),comparison between groups uses x~2 test.Measurement data uses(?xs),comparison between groups uses t test.P<0.05 was considered statistically significant.Results:The surgery was successfully completed in both groups,no perioperative death.In the thoracoscopy group,the operation time(135.2624.88)min,the intraoperative blood loss(115.9533.34)ml,the length of the surgical incision(9.512.56)cm,and the pain score on the first day after(2.261.01)minutes,drainage tube placement time(73.5219.58)hours,postoperative bed time(3.761.09)days,hospital stay(10.71.52)days,postoperative complications 7.20%.,the cost of hospitalization was(4.751.40)yuan.In the traditional surgery group,the operation time of(152.4427.51)minutes,the intraoperative blood loss(190.2645.08)ml,the length of the surgical incision(15.184.59)cm,and visual simulation on the first day after surgeryscore(3.151.05)points,drainage tube placement time(86.9217.22)h,postoperative bed movement time(4.871.26)d,hospital stay(14.23.45)d,postoperative complication rate 29.00%,hospitalization expenses(5.121.46)million yuan.Compared with the traditional surgery group,the thoracoscopic group has a lower surgical procedure time,intraoperative blood loss,surgical incision length,postoperative day 1 pain score,drainage tube placement time,There were significant differences in bed movement time,length of hospital stay,and postoperative complications(P<0.05).There was no significant difference between the two groups(P>0.05).Cortisol(111.8616.49)ng/ml,blood glucose(7.651.76)mmol/L,and C-reactive protein levels(53.8911.66)mg/L.Cortisol(127.0223.21)ng/mL,blood glucose(8.721.39)mmol/L,and C-reactive protein level(62.7514.76)mg/L.The stress response indicators of the patients in the two groups were statistically significant(P<0.01).Six months after discharge,follow-up was performed,and the overall evaluation was made based on the efficacy indicators.The thoracoscopic group was 90.50%better than the traditional surgery group 71.00%The differences were statistically significant(P<0.05).Conclusion:The application of thoracoscopy to rib internal fixation can shorten the length of hospital stay,without increasing the patient's financial burden,with minimal trauma,light pain,and rapid postoperative recovery.It has advantages in improving clinical efficacy,protecting physiological functions,and reducing body stress reactions.It has certain clinical promotion value.
