Objective : To investigate the risk factors,histopathological features of different degrees of atrophy and the value of endoscopy in patients with chronic atrophic gastritis.Methods: A total of 653 patients with chronic atrophic gastritis diagnosed by gastroscopy were selected,and then the basic information,gastroscopy reports,pathological biopsy reports,helicobacter pylori infection status and endoscopic mucosal characteristics of the patients diagnosed were analyzed.Results:1.In this study,485 patients with chronic atrophic gastritis diagnosed by pathological biopsy with an average age of 54.84±10.48 years,including 240 male patients with an average age of 53.89±10.81 years and 245 female patients with an average age of 55.77±10.07 years.2.The coincidence rate of chronic atrophic gastritis diagnosed by electronic gastroscopy and pathological diagnosis was 74.3%(485/653).3.Among the 485 patients in the chronic atrophic gastritis group,221 cases patients with helicobacter pylori infection(45.6%)and 172 cases patients aged60-80 years(35.5%).Among the 168 patients in the non-atrophic gastritis group,53 cases patients with helicobacter pylori infection(31.5%)and 40 cases patients aged 60-80 years(23.8%).The proportion of patients with helicobacterpylori infection and aged 60-80 years in chronic atrophic gastritis group was higher than that in non-atrophic gastritis group,and the difference was statistically significant(P <0.05).4.There were differences in gastric mucosa pathological results among chronic atrophic gastritis patients with different degrees of atrophy(P<0.05).The incidence of moderate and severe intestinal metaplasia and high-grade intraepithelial neoplasia in the moderately atrophic gastritis group was higher than that in the mildly atrophic gastritis group.The incidence of moderate and severe intestinal metaplasia and high-grade intraepithelial neoplasia was higher in the severe atrophic gastritis group than in the mild atrophic gastritis group.5.The status of helicobacter pylori infection is closely related to the mucosal manifestations(P<0.05).Patients with mucosal features including gastric xanthelasma,nodularity,enlarged fold,mucosal congestion and swelling,intestinal metaplasia are more common in helicobacter pylori infection.Those with fundic gland polyp,raised erosion,bile regurgitation,regular arrangement of collecting venules are more common in without helicobacter pylori infection.Conclusion:1.The age and helicobacter pylori infection are the risk factors of chronic atrophic gastritis.Intestinal metaplasia and intraepithelial neoplasia are positively correlated with the degree of atrophy.Such patients should be monitored regularly.2.The consistency between endoscopic diagnosis and pathological examination is good,,but there were still differences between them.3.The state of helicobacter pylori infection can be determined by observingthe characteristics of gastric mucosa under the electronic gastroscope.
