| Objective To collect clinical data of 109 patients diagnosed with multiple myeloma(MM)diagnosed in Gansu Provincial People’s Hospital from March 2013to December 2019,and explore plasma fibrinogen(FIB)and D-dimer(D-dimer,D-D)Clinical significance in the prognosis assessment of multiple myeloma.Methods 1.A retrospective analysis method was used to select 109 patients with MM who were diagnosed and diagnosed in the People’s Hospital of Gansu Province from January 2013 to December 2019.Collect clinical data,determine the optimal threshold of plasma fibrinogen(FIB)and D-dimer(D-D)according to the receiver operating characteristic curve(ROC)curve,and divide the patients into high FIB and D-D groups(FIB>4.34g/L,D-D>1.30μg/mL)and low FIB and D-D groups(FIB≤4.34 g/L,D-D≤1.30μg/mL),to assess the correlation between clinical indicators and plasma FIB and D-D.2.Use single factor analysis to evaluate the patient’s age,gender,clinical stage,immunotype,treatment plan and FIB,D-D,hemoglobin(HB),platelet(PLT),serum albumin(ALB),blood creatinine(Scr),biochemical The effects of blood glucose,blood calcium andβ2-microglobulin on progression-free survival(PFS)and overall survival(OS)of MM patients,and further meaningful variables were analyzed by Cox multivariate analysis.3.Kaplan-Meier survival analysis was used to compare the high FIB,D-D,PLT,Scr,and non-bortezomib treatment groups(FIB>4.34 g/L,D-D>1.30μg/mL,PLT>199.01×109/L,Scr>130.0μmol/L)and patients with low FIB,D-D,PLT,Scr,bortezomib(FIB≤4.34g/L,D-D≤1.30μg/mL,PLT≤199.01×109/L,Scr≤130.0μmol/L)Difference.Results 1.Plasma D-D is related to ISS stage and DS stage(p=0.032,p=0.001),plasma FIB is related to Scr value(p=0.005).2.Univariate analysis showed that D-D,PLT,Scr,ISS stage,bortezomib treatment group were the influencing factors of OS in newly diagnosed MM patients(p<0.05);serum free light chain was the influencing factor of PFS in newly diagnosed MM patients.Multivariate analysis results showed that D-D,PLT,Scr,and treatment options were independent risk factors for OS in newly diagnosed MM patients(p<0.05).3.Kaplan-Meier survival analysis showed that high FIB,D-D,PLT and Scr groups(D-D>1.3μg/mL,PLT>199.01×109/L,Scr>130μmol/L,non-bortezomib group)were lower than FIB,D-D and PLT,Scr group(D-D≤1.3μg/mL,PLT≤199.01×109/L,Scr≤130μmol/L,bortezomib group)OS is short(p=0.006,0.009,0.000,0.245 respectively).Conclusion 1.Among the included study parameters,higher D-D,PLT,and Scr among newly diagnosed MM patients are independent risk factors for patients’prognosis;Newly diagnosed MM patients with D-D≤1.3μg/mL,PLT≤199.01×109/L,Scr≤130μmol/L had longer OS.2.In this group of data,there is no correlation between fibrinogen and the patient’s survival time and disease progression. |
