Objective To evaluate the clinical effect of open reduction internal fixation ORIF and minimally invasive plate osteosynthesis MIPO in the treatment of humeral shaft fracture(AO C type),and to evaluate the impact of the two on the quality of life after operation,so as to provide data support for its subsequent clinical application. Methods From January 2010 to January 2019,94 cases of humeral shaft fracture(AO C type)were analyzed retrospectively.According to the operation mode, patients were divided into ORIF group(n = 51)and MIPO group(n = 43).The hospitalization days, hospitalization expenses, operation time, intraoperative blood loss, perioperative blood loss, postoperative pain score, shoulder mobility, elbow mobility, fracture healing time, postoperative fracture reduction, postoperative fracture healing X-ray score, postoperative complications(delay)were analyzed Union, nonunion, malunion, radial nerve injury, postoperative infection). At the same time,spss21.0 software was used to further analyze the collected case data. Results All patients had complete data and follow-up.1.MIPO group was significantly lower than ORIF group in terms of operation time, intraoperative blood loss and perioperative blood loss(P<0.05);2.MIPO group was significantly better than ORIF group in terms of postoperative pain VAS score, fracture healing time, postoperative shoulder joint function score, postoperative fracture reduction,X-ray score of fracture healing in 3,6 and 12 months The difference was statistically significant(P < 0.05);3.There was no significantdifference between the two groups in terms of hospitalization days, hospitalization expenses, postoperative elbow joint function score and postoperative 1-month X-ray score of fracture healing(P > 0.05);4.There were 2 patients in the ORIF group who had delayed fracture healing 8 months after operation, and all the patients in the MIPO group had good healing; One case of MIPO group had the clinical symptoms of radial nerve paralysis, two cases of ORIF group had the clinical symptoms of radial nerve paralysis, the other patients had other complications. Conclusion In this study, minimally invasive plate osteosynthesis is superior to incision reduction and plate internal fixation in the treatment of humeral shaft fracture(AOC type). It can reduce trauma, promote fracture healing and shorten healing time.Based on this study,it is suggested that minimally invasive plate osteosynthesis should be widely used in clinic.
