BackgroundThe pathological changes of benign prostatic hyperplasia(BPH)can cause male lower urethral syndrome(LUST),which is mainly manifested as progressive dysuria.In 1997,Homme and his colleagues surveyed Asian men and found that with the increase of age,there were moderate and severe micturition symptoms at 40 years old,and 56%at 70 years old.According to the latest epidemiology of benign prostatic hyperplasia in China,BPH is younger and the incidence rate is increasing year by year.The incidence rate is 50%when they are 50 years old in China,and the incidence rate is 80%in 70 years old.In clinical research,it is found that prostate volume does not necessarily being a positive correlation,and patients with small volume(?30ml)of prostate hyperplasia may also lead to progressive dysuria.After conservative treatment with drugs,the effect of such patients is generally poor,and further intervention is needed.The surgery is the preferred treatment plan,At present,the diagnosis and treatment of medium and large volume benign prostatic hyperplasia have been standard,but there is no unified diagnosis and treatment process and standard for small volume benign prostatic hyperplasia due to the complex etiology.If we do not pay attention to such patients,it will lead to misdiagnosis and delayed treatment.The patients with small volume of prostate hyperplasia combined with LUST in China Patients don't have a good treatment plan?The study aim to complete th assessment of the efficacy and safety of TUCBDP for small volume prostate(?30ml).As the same time,to demonstrate the safety and feasibility of this procedure and to make an initial assessment of effectiveness?methodsIt is a retrospective,non-randomised,single-centre trial with moderate-to-severe LUTS conducted during 2016-2019.Baseline,peri-operative and 6-month.urinary function data were collected after TUCBDP.Clinical data collected including:1 General data of patients:name,gender,age,height,weight 2.Observe the operation time(min),bleeding volume(ml),perioperative cardiopulmonary complications 3.International prostate symptoms scales I-PSS and quality of life before,1 month,3 months and 6 months Of life QOL,the maximum of urine quantity Qmax,residual urine volume RUV,retrograde ejaculation re,increased erectile dysfunction ed,bleeding,bladder spasm and urinary tract infection?ResultsIn all patients,21 of them underwent surgery under Lumbar epidural anesthesial in during 2016-2019.Finally,20 people get the complete data?The subject was 67.9 ±1.93 years old,and the prostate size was 23.9±3.56 ml.The preoperative international prostate symptom score(IPSS)was 19.7±3.6,and the QOL score was 4.0 ±0.65,Qmax was 4.0±1.9 mL/s,PVR was 69.8±78.3 ml,at the 6th postoperative month,IPSS was 7.9±1.7,QOL score was 1.6±0.69,Qmax was 15.7 ±1.76mL/s,PVR 9.7±7.3ml.Among them,IPSS score,QOL score,and PVR indicators decreased significantly,while Qmax increased significantly compared with preoperative,the difference was statistically significant(P<0.01).The intraoperative balloon dilation time was 18.3±4.2 minutes,and all operations were successful.During postoperative follow-up,1 patient developed transient urinary incontinence,and the patient recovered spontaneously within 1 month after surgery.One patient developed chronic urinary incontinence and he was relieved by pelvic floor muscle exercise within 3 months,and retrograde ejaculation occurred.In 1 case,1 patient developed bladder spasm,and no patient experienced postoperative bleeding or blood transfusion.ConlusionsThe transurethral balloon dilatation of the prostate(TUCBDP)is a surgical method for the treatment of small-sized benign prostatic hyperplasia.There are few postoperative complications,and it has advantages as a small volume of benign prostatic hyperplasia.Clinically,TUCBDP showed significant surgical efficacy during the 6 months of postoperative follow-up,but long-term follow-up studies are still required.In the actual surgical operation process,TUCBDP has a simple surgical operation and a little operation time.It is a minimally invasive treatment for prostate hyperplasia in line with China's national conditions,especially for the treatment of small prostate hyperplasia.TUCBDP has a broad prospect.
