Comparing The Efficiency Of Bacillus Calmette Guérin And Pirarubicin Intravesical Instillation In Patients With High-risk Non-muscle Invasive Bladder Cancer

Objective:To investigate the effect of intravesical instillation with BCG and pirarubicin on postoperative recurrence of high-risk non-muscle invasive bladder cancer.Methods:Retrospective analysis was performed to select 76 high-risk NMIBC patients who underwent transurethral resection of bladder tumor or laser resection of tumor in the department of urology,the first hospital of shanxi medical university from January 2016 to January 2018,and assisted postoperative intravesical instillation therapy.The cases were divided into Group A(BCG group,38 cases)and Group B(Pirarubicin group,38cases).All patients in group A were given BCG instillation after surgery,and those in group B were given pirrobuxacin instillation.Comparison of tumor recurrence and adverse reactions after intravesical instillation in two groups of patients.Results:The recurrence rates of group A and group B were 2.63%(1/38)and 10.53%(4/38)at 3 months after operation.There was no significant difference in the recurrence rates between the two groups(P = 0.165).The cumulative recurrence rates of patients in group A and group B at 6 months after operation were 5.26%(2/38)and 21.05%(8/38),respectively.The difference between the two groups was statistically significant(P =0.042).The cumulative recurrence rates of patients in group A and group B at 6 months after operation were 13.16%(5/38)and 34.21%(13/38),and the difference between the two groups was statistically significant(P = 0.031).The cumulative recurrence rates of patients in group A and group B at 12 months after operation were 18.42%(7/38)and42.11%(16/38),and the difference between the two groups was statistically significant(P = 0.025).The incidences of adverse reactions in group A and group B were 57.89%(22/38)and 26.32%(10/38),and the difference was statistically significant(P =0.005).Conclusion:Compared with THP intravesical instillation chemotherapy,BCG intravesical instillation treatment is more effective in preventing the 1-year recurrence-free rate after high-risk NMIBC surgery,which is worthy of clinical application and promotion.However,the adverse reaction rate of THP intravesical instillation chemotherapy was significantly lower than that of BCG intravesical instillation therapy,and the safety was higher.For patients unable to tolerate the adverse reaction after BCG instillation,THP intravesical instillation chemotherapy could be selected.
