Objective:To study the role of YAP protein in chronic rhinosinusitis with nasal polyps(CRSwNP),The expression of chronic rhinosinusitis without nasal polyps(CRSsNP)and normal nasal mucosa tissues as well as the correlation between the nuclear/qualitative expression of YAP protein in CRSwNP and CRSsNP tissues are analyzed its mechanism of action in the occurrence and development of chronic rhinosinusitis,lay a scientific experimental foundation for the pathogenesis of chronic rhinosinusitis diseases,and aim to treat chronic rhinosinusitis with YAP as a therapeutic target and provide new possibilities for clinical treatment.Methods:1.Patients with chronic rhinosinusitis and nasal polyps(without nasal polyps)admitted to the Department of Otolaryngology and Head and Neck Surgery,First Hospital of Shanxi Medical University from December 2018 to September 2019 were selected and functional nasal endoscopy was performed.During the operation,40 cases of clean nasal polyp apical tissues of patients with chronic rhinosinusitis and nasal polyps were taken as experimental group A and 40 cases of uncinate mucosa tissues as experimental group B;The control group was 20 cases of nasal septum deviation correction under functional nasal endoscope.2.Immunohistochemical SP method was used to determine the expression of YAP protein in nasal polyps and normal nasal mucosa.3.The expression of YAP protein was evaluated according to the staining intensity of each sample and the number of positive cells.The samples whose nuclear and / or cytoplasmic staining showed clear yellow to brown specific immunoreactive granules were judged to be positive.More than 5 high power visual fields(400 ×)were selected,and the staining intensity and area of each slice were observed and judged by semi-quantitative integration method.According to the staining intensity,it can be divided into four grades: colorless or negative staining(-),light yellow(+),light yellow(+),light yellow(+)and yellow(+).Yellow to brown yellow is strong positive(+),is 3 points.The number of stained cells ? 10% is 0,11% 25% is 1,26% 50% is 2,51% 75% is 3,76% 100% is 4;Finally,the score of each slice was calculated,and according to the average chromaticity score of each slice multiplied by the average percentage of stained cells,the final score was 0-2 points(-),3-4 points(+),5-6 points(+),7 points(+).4.The data were statistically analyzed by SPSS 21.0 software.Results:1.The expression of YAP protein in cytoplasm of CRSwNP,CRSsNP and normal nasal mucosa tissue is different.the expression results are divided into negative,weak positive,positive and strong positive according to staining standards.The expression of YAP protein in CRSwNP tissues showed 16,9,9 and 6 cases in the 4 groups respectively.The expression of YAP protein in CRSsNP tissues was found in 17,11,7 and 5 cases in 4 groups respectively.The expression of YAP protein in normal nasal mucosa tissues showed 14,4,2 and 0 cases in the 4 groups respectively.2.CRSwNP,CRSsNP and YAP protein in normal nasal mucosa tissues are expressed differently in nuclei.The expression results are classified into negative,weak positive,positive and strong positive according to staining standards.The expression of YAP protein in CRSwNP tissues was found in 17,11,7 and 5 cases in 4 groups respectively.The expression of YAP protein in CRSsNP tissues showed 18,12,6 and 4 cases in 4 Groups respectively.The expression of YAP protein in normal nasal mucosa tissues was found in 15,4,1 and 0 cases in 4 groups respectively.3.The positive expression of YAP protein in cytoplasm was positively correlated with that in nucleus.Conclusion:The expression of YAP protein in chronic rhinosinusitis was significantly higher than that in normal nasal mucosa.On the contrary,there was no obvious expression of YAP protein in cytoplasm and nucleus in the epithelial cells and glandular epithelial cells of normal nasal mucosa.There was no significant difference in the expression of YAP protein between CRSwNP nasal polyps and inflammatory mucosa in CRSsNP.This may be because YAP not only participates in the development of persistent inflammation in the occurrence and development of CRS,but also promotes tissue remodeling by promoting EMT and activating epithelial stem cells.The expression of YAP protein in nucleus and cytoplasm is positive,and there is a positive correlation between them.YAP protein gradually accumulates in the cytoplasm,and its excessive accumulation can cause YAP itself to transfer to the nucleus.YAP after entering the nucleus can activate transcription factors and activate them,resulting in a series of proliferation-promoting effects.YAP is involved in the formation mechanism of chronic rhinosinusitis,which leads to tissue remodeling by promoting the development of chronic inflammation and regulating cell proliferation.
