Objective:Through the monitoring of somatosensory evoked potential(SEP)in patients with severe craniocerebral injury(sTBI),the value of its clinical application in critically ill patients and the correlation between early monitoring results and the prognosis of patients were discussed.Methods:The clinical data of 52 patients with sTBI who met the inclusion criteria from February to October 2019 in the Department of Neurosurgery,Shanxi Bethune Hospital were collected.After admission,the SEP was recorded at the same time every day within1,3,5,and 7 days,and the GCS score was recorded at the same time.For the SEP monitoring results,refer to the classification standards proposed by Houlden et al.The termination time of prognosis judgment is 3 months after the onset.According to the results of follow-up,GOS score and Barthel index were used for evaluation.The correlation analysis of SEP classification,GOS score and Barthel index was performed by bivariate Spearman correlation factor analysis test,and the difference was statistically significant with P<0.05.The N20 peak and latency of the good prognosis group and the poor prognosis group were tested by t test,and the difference was statistically significant with P<0.05.Results:SEP monitoring of patients with sTBI on the 1st,3rd,5th,and 7th days afteradmission was significantly correlated with the prognosis of the patients.The correlation coefficient was the highest on the 7th day and had statistical significance(P<0.001),suggesting that the prognosis was the most relevant.The correlation between 3d-7d potential classification and prognosis is good.The N20 peak and latency of the good prognosis group and the poor prognosis group were statistically significant.Conclusion:1 The results of somatosensory evoked potential monitoring have a positive correlation with the prognosis of the patient,and the somatosensory evoked potential classification on the 7th day is the most suggestive of the patient’s prognosis;2 The correlation between 3d-7d internal potential classification and prognosis is high,and it is recommended as the prognosis judgment time;3 The decrease in N20 amplitude and the prolonged incubation period suggest a poor prognosis.
