Objective To find out the influence factors of proteinuria elimination time in children with primary nephrotic syndrome,direct clinical treatments better than before,reduce renal tubular damage and improve the prognosis of the patients.Methods The children who were first diagnosed as PNS in our hospital from January2013 to October 2017 were selected as the study subjects.And then divided into groups by the proteinuria elimination time.Collect the subjects general conditions and laboratory indicators.We use the Mean ± standard deviation to describe the measurement data with normal distribution,and use the one-way anova analyze the difference between groups.We describe the measurement data with non-normal distribution by median and quartile,and we use the kruskal-wallis H to describe the difference between groups.The counting data were described by the number of cases and percentage,the difference among groups was analyzed by chi-square test,and the Fisher exact probability method was used for failure to meet the chi-square test conditions.Finally,multivariate ordered logistic regression analysis was performed on the statistically significant variables after univariate analysis.Results We analyze 185 included patients,the median age of onset in all cases was 4years.,the quartile spacing is 2-8 years,the peak period of disease is in the preschool age.There was no significant difference in the incidence gender of PNS(P>0.05).There were differences in age between the four groups,and the results is p=0.015,which means the difference was statistically significant.For the counting data:the qualitative of urine protein?the degree of hematuria?the situation of infection?the medication compliance and the days from the symptoms to oral hormone(<10days and ?10days),and the results indicate:Thedifferences in the days from the symptoms to oral hormone ?medication compliance and the qualitative of urine protein were statistically significant(p<0.05).And for measurement data:age?albumin(ALB)?triglyceride(TG)?serum creatinine(SCr)?blood urea nitrogen(BUN)?the quantity of 24-hour urinary protein?leukocyte(WBC)?platelet(PLT)?Immunoglobulin A(IgA)?Immunoglobulin M(IgM)?Immunoglobulin G(IgG)?activated partial thromboplastin time(APTT)?erythrocyte sedimentation Rate(ESR)?prothrombin time(PT)?C-reaction protein(CRP)?addiment C3?the days of the first time proteinuria elimination after oral hormones?hemoglobin(HGB)?total protein(TP)?total cholesterol(TC)?fibrinogen(FIB),we use the Mean ± standard deviation to describe the measurement data with normal distribution,and use the one-way anova analyze the difference between groups.The results show: the differences in triglyceride?the quantity of 24-hour urinary protein?leukocyte?Immunoglobulin A?the days of the first time proteinuria elimination after oral hormones?total cholesterol were statistically significant(p<0.05).We make the indicators of statistical significance in univariate analysis(age?triglyceride?total cholesterol?the quantity of 24-hour urinary protein ?leukocyte ?Immunoglobulin A?the days of the first time proteinuria elimination after oral hormones?total protein?the days from the symptoms to oral hormone?the medication compliance and the qualitative of urine protein)do the parallelism test,and the indicators which satisfies the parallelism test to carry out multiple ordered logistic analyze,the results shows that for every time we increase by 1 in the time it takes for the days of the first time proteinuria elimination after oral hormone,we think the OR value of proteinuria transitioning time is 1.261 times of the original(95% CI:1.185-1.340),which means the difference was statistically significant(P<0.001).Comparing with the days from the symptoms to oral hormone ?10days,the OR value of the days from the symptoms to oral hormone <10days is 0.260(95% CI:0.125-0.542),which means the difference was statistically significant(P<0.001);Comparing with the good compliance,the OR value of the bad compliance is 10.350(95% CI:4.302-24.903),which means the difference wasstatistically significant(P<0.001);we divided the qualitative of urine protein into groups pro1+,2+,3+,4+,comparing with the patients with pro 4+,the patients with pro 1+ whose proteinuria elimination time increased 1 level,the OR value was 0.16 times higher than the original value,which means the difference was statistically significant(P=0.043).Conclusion 1.The median age of onset of PNS in the collected cases was 4 years old,and the peak age of onset was pre-school age,and there is no difference in gender in each group.2.the longer the days from the symptoms to oral hormone?the longer the days of the first time proteinuria elimination after oral hormones?the bad compliance and the higher qualitative of urine protein are the risk factors for the proteinuria elimination time.
