Objective Three different burn wound dressings were used to treat deep second burn wounds,comparing the effects of wound healing,wound infection,painful stimulation and post-scarring during dressing change to investigate the effects of basic fibroblast growth factor(b FGF)combined with porcine acellular dermal matrix on the deep second degree burn wounds.Methods From July 2017 to November 2018,90 patients with deep second degree burn were selected according to the inclusion and exclusion criteria.They were coded according to the order of mission and randomly divided into 3 groups,30 in each group.Using b FGF-porcine acellular dermal matrix dressing(group A),porcine acellular dermal matrix dressing(group B)and b FGF impregnated gauze(group C)to treat burn wound,observing and recording wound healing rate,wound healing time,wound infection status,pain level and other indicators.Six months after the injury were cured,the patients in each group were followed up to observe the scar hyperplasia and pigmentation.Result 1.The average healing time of wounds in group A,B and C was 15.61±3.73 days,18.23±4.04 days,24.37±4.49 days respectively.The healing time of group A was shorter than that of group B and C,and the healing time of group C was longest.There were significant differences in the healing time between each two groups.2.Comparing the wound healing rates of 7th day,14 th day and 21 st day,the wound healing rate of group A was higher than other groups,and there is significant difference between each two groups.3.The number of positive bacterial cultures and the bacterial infection rate of wounds on the first day showed that there was no significant difference in the scores of A,B and C groups.The results of bacterial culture positive and infection rate of wounds among three groups of 7th day,14 th day,21 st day showed no significant difference between A and B groups.In addition,the results of bacterial culture positive and infection rate of wounds of group A and group B were lower than group C,and the difference was statistically significant.4.Differences in pain scores of three groups on the first day are no statistical difference.Through the follow-up of 7th day,14 th day and 21 st day,the frequency of dressing changing and pain scores in group A and group B were lower than those in group C,and the difference between the two groups was statistically significant compared with group C.5.After six-months follow-up,the Vancouver Scar Scale of three different treatment methods of wound healing shows that the scar hyperplasia in group A are relatively flat,light and the joint activity is good.The rate of scar hyperplasia was highest in group C,the hyperplasia and pigmentation of scars were heaviest.Conclusion Using basic fibroblast growth factor combined with porcine acellular dermal matrix dressing can accelerate the healing rate of wounds,improve anti-infective ability,greatly reduce the pain during dressing change and complications.In addition,the proliferation and pigmentation of scar are significantly mitigated.
