Research Objective:Burns are common skin injuries and are a serious worldwide health problem with an average annual incidence of about 4-12 ‰.Scar hyperplasia, scar contracture and joint dysfunction caused by severe burns will have a great impact on the patient's physical and mental health, not only for the individual patient, but also for the patient's family, which will cause huge damage and a huge burden.Especially in children with burns. The proportion of children with burns in burn patients gradually increased, accounting for 32% to 43%.Due to some characteristics of pediatric skin anatomy and physiology. At the current stage, the basic and clinical application research level of burn treatment in China is developing rapidly, and the clinical treatment level of burns in China is also at the forefront of the world.Promoting wound healing and preventing and controlling burn wound infection are two key factors in the treatment of burn wounds.Various types of silver-containing dressings and various growth factors have not only been limited to laboratory research, but are being widely used in various types of wounds, including burn wounds. This study was to investigate the clinical efficacy and adverse reactions of nano-silver dressing combined with basic fibroblast growth factor in the treatment of children with second degree burn wounds, and provide clinical evidence for its rational application.Research method:This study selected 88 children with second degree burns who were admitted to the Department of Burn and Plastic Surgery of Yantai Yuding Hospital from January 20 to 2018.All patients were treated with Xinjie Erfeng, cut blister, cleaned vesicular skin, intramuscular injection of tetanus antitoxin, appropriate amount of fluid, and prevention of infection.According to the different external drugs, they were divided into nano silver dressing group(n=22),basic fibroblast growth factor group(b FGF group)(n=20),combined group(n=26)and blank control group(n= 20).The 4 groups of children were changed according to the penetration of the dressing, and the wounds were healed or the wounds were healed and discharged. The wound healing time and infection rate of each group were observed, and the pigmentation and scar hyperplasia after wound healing were followed up.Research Results:The healing time of burn wounds in different treatment groups was different: combined group <nano silver dressing group<alkaline fibroblast growth factor group< blank control group;wound infection rate of different treatment groups was not statistically different;combined group had low pigmentation rate In the other three groups, the difference was statistically significant(P <0.05);the scar of the combined group was significantly better than the other three groups, and the difference in the Vancouver scar scale(VSS)score was statistically significant(P<0.05).Conclusion:Nano-silver dressing combined with basic fibroblast growth factor can effectively shorten the healing time of wounds and reduce the pigmentation and scar formation after wound healing. It is worthy of clinical application.
