Objective:By comparing and analyzing the clinical data and treatment of thyroid cancer patients undergoing total thyroidectomy(TT)and subtotal thyroidectomy,we explored the effect of TT on short-term and long-term complications of thyroid cancer and surgical-related conditions influences.Methods:A retrospective analysis of the clinical data and treatment status of 48 patients with thyroid cancer who underwent TT surgery and subtotal resection at the Department of Otolaryngology Head and Neck Surgery at the First Affiliated Hospital of Nanchang University from October 2017 to October 2018.Among them,the TT operation was the research group,and the subtotal resection was the conventional group.Then,the serum calcium levels,recent surgery-related complications,long-term bad situation,and surgery of the 2 groups of patients before and after operation on day 1,day 5 and year 1 Analysis of treatment-related conditions.In order to explore the effect of TT operation on the long-term complications of thyroid cancer and surgery-related conditions.Results:1.General clinical data: There is no significant difference in general clinical data(gender,age and pathological type)between the two groups,P> 0.05,the difference is not statistically significant.2.Serum calcium levels before and after surgery: There was no statistically significant difference in serum calcium ion levels between the two groups of patients before operation(P>0.05).After the operation,the two groups of patients were compared on the first day,the fifth day and the first year Statistical analysis of serum calcium levels found that the two groups had differences in serum calcium levels at 1 day and 5 days after operation,the difference was statistically significant(P<0.05)and the study group was lower than the conventional group;but At 1 year after surgery,there was no statistically significant difference in serum calcium ion levels between the two groups(P>0.05),and all reached normal serum calcium levels.3.Recent complications: Recent complications include temporary hoarseness,temporary hypoparathyroidism,and postoperative secondary bleeding.After surgical treatment,there were 4 cases of temporary hoarseness in the study group.Among them,2 patients had unilateral recurrent laryngeal nerves wrapped in tumors.It was difficult to separate them during the operation.The recurrent laryngeal nerves were permanently severed.9 cases had temporary parathyroid Hypogonadism and 1 postoperative secondary bleeding,1 case of the control group had transient hoarseness,2 cases of temporary hypoparathyroidism and 2 cases of secondary bleeding,and 2 groups had temporary parathyroid glands The number of people with hypofunction was significantly different,and the study group was more than the control group,the difference was statistically significant(P<0.05),temporary hoarseness and postoperative secondary bleeding were not significantly different between the two groups,not Statistical significance(P>0.05),The difference between the total incidence rate was obvious and statistically significant(P<0.05).4.Long-term bad situation: Long-term adverse conditions include permanent hoarseness,permanent hypoparathyroidism,and bad nodules in the neck.After a recovery period of at least 6 months after surgery,there was 1 case of permanent hoarseness in the study group,2 cases of permanent hypoparathyroidism,and no cases of permanent hoarseness and permanent parathyroid gland in the control group Functional decline,no statistical difference between the two groups by statistical analysis(P>0.05);1 year later,by cervical ultrasound or neck CT review,6 cases in the control group had Suspicious nodules(Such as nodules in the remaining glands,lymph nodes in the central area,etc),the study group No abnormalities were found in,the difference between the two was statistically significant(P<0.05).5.Surgery related situation: Analysis of the surgical treatment of the two groups of patients found that compared with the control group,the patients in the study group had a shorter time to complete the operation,less intraoperative blood loss,less drainage tube drainage,shorter hospital stay,and number of lymph node dissections More,the difference is statistically significant(P<0.05).Conclusion:Although the incidence of recent complications of TT surgery in the surgical treatment of thyroid cancer is high,the long-term adverse conditions are better.At the same time,it can also improve the related conditions of surgical treatment,reduce the amount of intraoperative bleeding,shorten the length of hospitalization,and survive The improvement of quality has a positive significance,so for surgeons with rich experience in surgery,they can choose the corresponding surgical method according to the actual situation of the patient.When conditions permit,they can preferentially choose total thyroidectomy.
