Objective:To investigate the relationship between the levels of hemoglobin a1c(HbA1c)and interleukin-6(il-6)and vascular endothelial growth factor(VEGF)in aqueous humor in diabetic patients with cataract before phacoemulsification.To discuss the role of preoperative HbA1 c and perioperative blood glucose level in predicting the inflammatory response of anterior ganglitis after cataract surgery in diabetic patients.so as to lay a theoretical foundation for correctly understanding the value and significance of HbA1 c and blood glucose monitoring in cataract surgery in diabetic patients.Method:In this study,patients who were diagnosed with cataract and underwent phacoemulsification surgery in our hospital from September 2019 to February 2020 were collected,and the subjects who met the inclusion criteria were divided into the diabetic group and the non-diabetic group.The diabetes group was divided according to HbA1 c level and FBG on the day of operation: group A had HbA1 c < 7.0% and FBG < 7.0mmol/L.Group B had HbA1 c < 7.0% and FBG?7.0mmol/L.Group C had HbA1c?7.0% and FBG<7.0mmol/L.In addition,15 cases of age-related cataract patients without diabetes were randomly selected as the experimental control group.During the operation collect aqueous humor,the contents of il-6 and VEGF in aqueous humor were determined by enzyme-linked immunosorbent assay(ELISA).The visual acuity,intraocular pressure and degree of anterior segment inflammation were analyzed and compared among the four groups.The correlation between preoperative HbA1 c level,daily FBG and the contents of il-6 and VEGF in aqueous humor was analyzed.Results:1.The levels of il-6 and VEGF in aqueous humor were compared between the experimental group(diabetic group)and the control group(age-related cataract patients without diabetes): group B and group C were significantly higher than those of the control group,and the difference was statistically significant;group A were not statistically significant compared with the control group.2.The levels of il-6 and VEGF in aqueous humor of each experimental group were compared:group C were significantly higher than that of group A and group B,and the difference was statistically significant;There was no significant difference between group A and group B.3.Comparison of anterior chamber reaction degree of four groups of patients after operation:group C was more severe than that of the control group,and the difference was statistically significant;There was no significant difference between group A with group B and group B and group C and the control group.4.Comparison of visual acuity and intraocular pressure on the first day after surgery in four groups:the control group was significantly better than that of group B and group C,and the difference was statistically significant;the difference between the control group and group A was not statistically significant.group A was significantly better than that of group C,and the difference was statistically significant;there was no significant difference between group A and group B and between group B and group C.There was no significant difference in intraocular pressure between the four groups on the first day after surgery.5.Correlation analysis of HbA1 C level with il-6 and VEGF levels:it was found that the level of HbA1 c was significantly correlated with the levels of il-6 and VEGF in aqueous humor at the level of P=0.01,and the correlation was positive.Conclusion:1?The level of HbA1 c before cataract surgery in diabetic patients was positively correlated with the level of il-6 and VEGF in aqueous humor,that is,the higher the level of HbA1 c before surgery,the higher the level of il-6 and VEGF in aqueous humor,which would increase the risk of postoperative anterior segment inflammationin these patients.2 ? In order to provide the safety of cataract surgery for diabetic patients,especially during daytime surgery,it is recommended to control the level of HbA1 c within the appropriate range to improve the safety of surgery.
