Objective Total Knee Arthroplasty(TKA)is the best treatment for severe knee osteoarthritis,which can reduce the pain of knee joint and joint movement disorder,and improve the quality of life of patients.However,patients still need a long period of rehabilitation exercise to reduce dysfunction and promote rehabilitation.The compliance of postoperative functional exercise of TKA patients is low,which leads to the knee rehabilitation fails to achieve the expected effect.Information-Motivation-Behavioral Skills Model(IMB)is mainly used to predict preventive health behaviors and implement health education,which belongs to the model of changing human health-related behaviors.IMB model divides the factors influencing behavior change into three aspects:information,motivation and skills.This study explored the application mode of IMB theory in functional exercise of patients after total knee arthroplasty,enriched the application of IMB theory in nursing;through IMB theory,a more comprehensive,specific and systematic functional exercise application module was developed to guide clinical nurses to improve the quality of health education,the effect of functional exercise,the function of knee joint and the quality of life of patients after total knee replacement.Methods This study is a kind of experimental study72 patients underwent total knee replacement were selected from June 2018 to March 2019 in a hospital of**with a convenient sampling method,meeting the inclusion and exclusion criteria.Among them,36 patients included from June 2018 to October 2018 were the control group and 36 patients included from November 2018 to March 2019 were the experimental group.The subjects in the control group received regular education during hospitalization.On the basis of the control group,the information-motivationbehavior skill model was used in the experimental group,patients underwent total knee replacementwere given intervention in information,motivation and behavioral skills in functional exercise.In terms of information intervention,a manual of functional exercise was made;in terms of motivation intervention,a semi-open interview outline was made;in terms of behavioral skills intervention,explanation and demonstration were adopted.At the time of admission,One week,one month and three months after surgery,the patient's general information questionnaire,number rating scale,range of motion and hospital for special surgery knee scorewere respectively used to investigate or measure the general data,pain of the knee,the knee joint activities of the research object and function of the knee joint in the two groups.The MOS item short from health surveywere used to assess the quality of life in the two groups of the object at the time of admission and three months after surgery.Results A total of63 cases were collected,including 32 cases in the experimental group and 31 cases in the control group.Results showed that(1)the ROM of the experimental group was higher than that of the control group at the same period at 1 and 3 months after surgery(P<0.05).(2)NRS of the experimental group was lower than that of the control group at 1 and 3 months after surgery(P<0.05).(3)the HSS of the experimental group were significantly higher than those of the control group at 1 and 3 months after surgery(P<0.05).(4)the total mean score,physiological function dimension score,physical pain dimension score and social function dimension score of the SF-36 in the experimental group was higher than that in the control group at the same period at 3 months after surgery(P<0.05).Conclusion Intervention based on information-motivation-behavior skill model for functional exercise after total knee replacement can improve the range of motion,reducepain,improve knee function,improve quality of life,and promote early recovery.
