| Background:Along with the rapid development of domestic economy,the public dietary structure has made corresponding changes,resulting in an increase in the number of diabetic patients annually.Once the blood sugar is not controlled,there will be a series of related complications.diabetic nephropathy(DN)is one of the most important and serious complications.In the early treatment of DN,ACEI or ARB can repair kidney damaged tissues and delay disease progression to a certain extent on the basis of controlling blood sugar.But the clinical effect is limited.However,traditional Chinese(TCM)medicine has a long history of understanding DN and has outstanding curative effect in clinical application.Through a large number of clinical accumulation,we believe that the early stage of DN is mostly due to deficiency of both qi and yin,and blood stasis hinders kidney collaterals.Therefore,we have formulated the method of Yiqiyangyintongluo Recipe to treat DN patients with deficiency of qi and yin and Stasis.Preliminary small sample studies have shown that this method can improve clinical symptoms,clinical indicators,reduce blood sugar,urine microalbumin,renal function,etc.Objective:To further determine the efficacy and safety of Yiqiyangyintongluo Recipe by observing the 24-hour urine protein quantity,24-hour urine creatinine,IL-6 and blood creatinine levels,pathological changes of residual kidney tissue and changes of kidney tissue TNF-??TGF-?1 in rats,so as to provide laboratory data for Yiqiyangyintongluo Recipe in treating DN,and to provide evidence-based basis for further study of its mechanism.Methods:SD male rats were randomly divided into 5 groups according to the method of number table:Normal group,model group and TCM group(low dose group,medium dose group and high dose group)with 10 rats in each group‘Except for the normal group,SD rats in other groups were fed with high sugar and high fat diet combined with STZ intraperitoneal injection to prepare DN rat models.After 8 weeks of intervention,24-hour urine protein,24-hour urine creatinine,blood sugar,EL-6and blood creatinine levels were measured in the model group and the TCM group.The pathological changes of renal tissues were observed by HE staining,and the expression of TNF-??TGF-?1 in kidney tissue were detected by ELISA.Results:After modeling,the 24-hour urine protein,24-hour urine creatinine,blood sugar and blood creatinine levels in renal tissue of rats in model group and TCM group were significantly higher than those in normal group(P<0.05).After treatment,the levels of 24h urinary protein,24h urinary creatinine,blood sugar,IL-6 and serum creatinine and the expression of TNF-??TGF-?1 in renal tissue in the TCM group were significantly lower than those in the model group(P<0.05),and were positively correlated with the dosage,but still higher than those in the normal group.After treatment,the pathology of nephropathy in the TCM group was significantly improved compared with that in the model group,the basement membrane was thickened,the mesangial matrix was slightly increased,and the infiltration of glomerular inflammatory cells was significantly reduced.Conclusion:Yiqiyangyintongluo Recipe can control blood sugar,reduce urinary protein excretion,serum creatinine level and increase urinary creatinine excretion in DN rats,which may reduce IL-6 levels,delay the progress of DN by inhibiting the expression of TNF-??TGF-?1 in renal tissue. |
