| Objective: To observe the clinical effect of Tiaozhi Jiangtang Tablet on reducing blood sugar and glycosylating red eggs in obese type 2 diabetes patients with phlegm-blood stasis syndrome,and then observe the changes of lipopolysaccharide,TNF-a and IL-6 before and after treatment.To understand the dysfunction of intestinal flora in patients with type 2 diabetes through the level of lipopolysaccharide,and finally explore whether Tiaozhi Jiangtang Tablet can achieve the goal of lowering blood sugar by regulating intestinal flora.Methods: Using randomized,controlled and parallel experimental design method,obese patients with type 2 diabetes mellitus of phlegm and blood stasis syndrome were randomly divided into treatment group(40 cases in Tiaozhi Jiangtang tablet group)and control group(35 cases in control group).The clinical symptoms,signs and laboratory indicators were observed before and after treatment for 12 weeks.The curative effect was evaluated by the criterion of clinical efficacy.The TCM syndrome score and related laboratory indexes were analyzed by IBM SPSS 21.0.Results:1.Scoring of TCM syndromes After treatment,the total effective rate was 85.00% in the treatment group and77.14% in the control group.Compared with before treatment,both groups could improve the clinical symptoms and signs of patients(P < 0.05),and the clinical effect of the treatment group was better than that of the control group(P < 0.05).2.Blood sugar and glycosylated hemoglobin levels Compared with before treatment,the levels of fasting blood sugar,2h postprandial blood sugar and glycosylated hemoglobin in the two groups were improved(P < 0.05),but there was no significant difference between the two groups(P > 0.05),indicating that the treatment group was better than the control group in lowering blood sugar level.3.Indicators of lipid metabolism Compared with before treatment,the indexes of reducing TG,TC and LDL were improved in both groups(P < 0.05),but there was no statistical difference betweenthe two groups(P > 0.05).The therapeutic effect of the treatment group and the control group was similar in reducing TG,TC and LDL,and the level of free fatty acid was improved in both groups(P < 0.05).05),indicating that the therapeutic effect of the treatment group on reducing FFA is better than that of the control group.4.Improvement of obesity In terms of improving obesity and weight loss,BMI and abdominal circumference were improved in both groups(P < 0.05)compared with those before treatment,and there was a significant difference between the two groups(P < 0.05),indicating that the weight loss effect of the treatment group was better than that of the control group.5.Insulin resistance recovery To improve insulin resistance,compared with before treatment,the levels of HOMA-IR in both groups decreased(P < 0.05),and there was a significant difference between the two groups(P < 0.05),indicating that the therapeutic effect of the treatment group was better than that of the control group.6.Inflammatory cytokines For TNF-alpha and IL-6,the level of TNF-alpha in the treatment group decreased significantly(P < 0.05);the level of TNF-alpha in the control group did not improve significantly(P > 0.05),indicating that the therapeutic effect of the treatment group on reducing TNF-alpha was better than that of the control group;compared with the treatment group,the level of IL-6 in both groups was improved(P < 0.05).The difference between the two groups was statistically significant(P < 0.05),indicating that the therapeutic effect of the treatment group on reducing IL-6 was better than that of the control group,and further indicating that the therapeutic effect of the treatment group on improving chronic low-grade inflammatory response was better than that of the control group.7.Improvement of intestinal flora disorder Compared with before treatment,LPS of the two groups were improved(P < 0.05).There was significant difference between the two groups(P < 0.05).It showed that the effect of treatment group on reducing LPS was better than that of control group.It further showed that the effect of treatment group on improving intestinal flora disorder was better than that of control group.Conclusion: Tiaozhi Jiangtang Tablet is superior to the control group in improving the symptoms and signs of obese type 2 diabetes mellitus patients with phlegm-stagnation syndrome,reducing TNF-a,IL-6,LPS,FFA,HOMA-IR and so on.It has the same effect in lowering blood sugar level,regulating TG,TC and LDL as the control group.It can be seen from this study that Tiaozhi Jiangtang Tablets cannot only reduce blood sugar level and improve clinical symptoms in patients with T2 DM,but also reduce the level of inflammatory cytokines,regulate intestinal flora disorders and improve insulin resistance,thus exploring the hypoglycemic mechanism of Tiaozhi Jiangtang Tablets,and providing a preliminary research basis for TCM hypoglycemic drugs. |
