| Objective:To explore the application effect evaluation of the management of hypertension patients in community based on the model of visual network + We Chat platform.Methods:The patients with hypertension in xiangwei huayuan community and heyetang community under the jurisdiction of hunan people's hospital were selected for study on December 1,2019 solstice and January 31,2020.A total of 115 hypertensive patients conforming to this study were randomly selected from conventional community hypertension management patients in Hunan Primary Health Information System 3.0,and the control group and intervention group were lined up.There were 60 cases in the control group and 55 cases in the intervention group.The control group received routine hypertension management during the observation period,including blood pressure,sodium intake standard,exercise standard,body mass index,knowledge,attitude and practice scale score,quality of life score.The intervention group managed the above projects comprehensively through the visual networking + We Chat platform,conducted consultation with the visual networking,health education related to hypertension on the visual networking + We Chat group platform,and provided personalized guidance services.After 3 months,the systolic blood pressure,diastolic blood pressure,sodium intake and other relevant data of the two groups of patients with hypertension were collected and statistically analyzed.Results:1.For the comprehensive management of hypertension in the community through the model of visual network + We Chat platform,the systolic blood pressure of patients in the intervention group decreased from 142.6±10.1mm Hg before intervention to 138.2±8.3mm Hg,with a decrease rate of 4.5mm Hg,which was significantly higher than that of the control group,with statistically significant difference(P<0.05).The change of systolic blood pressure in the control group was not statistically significant(P >0.05).2.The achievement rate of low sodium intake in the intervention group increased from 25.5% to 49.1% after intervention;In the control group,the compliance rate of low sodium increased from 23.3% to 30.0%.Chi-square test indicated statistically significant difference(P<0.05).3.Three months after intervention,the scores of the hypertension knowledge,attitude and practice scale in the intervention group were significantly higher than those in the control group in terms of knowledge evaluation,attitude and belief evaluation and compliance evaluation,with statistically significant differences(P<0.05).In terms of the evaluation of physical health dimension and mental health dimension,the SF-36 score of the intervention group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).Through the model management of visual network + We Chat platform,the intervention group was superior to the control group in terms of drug adherence rate,drug use method,and knowledge received,etc.Conclusion:1.It is feasible for hypertension patients in the community to use the visual network + We Chat platform mode for comprehensive hypertension management.2.The application of the visual network + We Chat platform mode to manage the blood pressure of hypertension patients is conducive to the treatment of hypertension,and is conducive to solving the personalized problems of hypertension patients,so as to improve the treatment effect of hypertension patients in the community. |
