Part one:Literature Study on the Characteristics of Numbness SyndromeObjective:To summarize the characteristics of TCM syndrome elements of numbness(polyneuritis)and the distribution regularity of syndrome types by studying the relevant literature of numbness(polyneuritis),so as to provide guidance for clinical differentiation and treatment of numbness.Method:From the literature database of China Knowledge Network,Wanfang,Weipu, etc.,select the literature on the syndrome differentiation of numbness(multineuritis)from May 20 th to May 31 st,and select the documents that meet the requirements according to the inclusion and exclusion criteria.Type and syndrome, establish a database, use the frequency analysis method to statistically analyze the data, and use the basic theory of Chinese medicine to comprehensively analyze the statistical results.Result:A total of 4249 patients with limb numbness were included in 27 articles which met the inclusion criteria. There were 42 syndromes and 23 syndromes, including 16 disease-related syndromes and 7 disease-location syndromes.According to frequency analysis, there are 16 types of syndromes with numbness frequency more than 2%,among which phlegm and blood stasis blocking collateral type is the most frequent.Numbness location syndrome elements include meridians, kidneys, spleen, liver, lung, blood vessels and stomach, among which meridians are the most frequent. The pathogenic factors are blood stasis,Yin deficiency,Qi deficiency, phlegm, cold and Yang deficiency, among which blood stasis,Yin deficiency,Qi deficiency and phlegm rank the top four.Solid venereal syndrome elements accounted for 64.51% of the total disease, while deficiency venereal syndrome elements accounted for 35.49% of the total disease venereal syndrome elements. The single disease-site syndrome elements account for 30.22% of all cases, and the single disease-site syndrome elements of meridians and collaterals account for the highest frequency;the complex disease-site syndrome elements frequency is 9.98%,and the disease-site syndrome elements composed of liver and kidney are the most frequent;the two-factor disease-venereal syndrome elements combination of complex disease-venereal syndrome elements has the highest frequency, accounting for 80.09% of the total complex disease-venereal syndrome elements, and phlegm + blood stasis has the highest frequency in the twofactor combination. In conclusionIn this literature study, the main syndromes of numbness are phlegm and blood stasis,Yin deficiency and blood stasis,Qi deficiency and blood stasis, liver and kidney deficiency,qi stagnation and blood stasis,wind-cold obstruction of collaterals,Yang deficiency and blood stasis,Qi-Yin deficiency,dampness-heat invasion,qi-blood deficiency and so on.The disease location syndrome elements and disease-related syndrome elements are extracted from the syndrome types, among which meridians and collaterals are the most important ones. The combination of phlegm and blood stasis syndrome elements is the most common combination of disease syndrome elements of numbness, and the mixture of deficiency and excess is the pathological feature of numbness.Part two: Clinical study on the distribution pattern of numbness syndromeObjective: To analyze the distribution characteristics of numbness(polyneuritis)symptoms and TCM syndromes by frequency analysis, factor analysis and cluster analysis.Method:A retrospective study was carried out in this study. The symptoms of numb patients collected by hospitals in the collaborative group of numb(polyneuritis)clinical pathway formulation were counted.Excel table was established.Data were analyzed by SPSS25.0 statistical software. The distribution characteristics of syndromes of numb(polyneuritis)were obtained by frequency, factor analysis and cluster analysis.Basic syndrome type.Result:A total of 283 patients were included in this study,of whom 40.99% were males and 59.01% were females. The ratio of male to female is about 1:1.43.The age of onset of the disease is concentrated between 50 and 80 years old, accounting for 79.50% of the total, with an average age of 61.7 years. The majority of the patients whose course of disease is less than 1 month and 1-5 years are 27.56% and 30.04% respectively.Frequency statistics of the four diagnostic data of 283 patients showed that the symptoms with a frequency of more than 10% included needling sensation, ant movement sensation, hypoesthesia, pain, dry mouth, constipation knot,five-heart upset, dark complexion, night sweat, skin armour error, stool, mouth sticky, chest tightness, waist and knee soreness, heavy head wrapping, worsening after fatigue, dim complexion, dizziness, socks feeling, fatigue.Dizziness, shortness of breath and laziness, insomnia and dreaminess, bitter mouth, pale complexion, dishonorable claws, and lack of acceptance. In tongue picture, the highest probability of occurrence of tongue bruises is 34.28%;the highest probability of occurrence of tongue coating is thin and white, accounting for 33.22%;the highest probability of occurrence of tongue texture is red, accounting for 32.51%;in pulse picture, the probability of occurrence of pulse astringency, smooth and astringent is 29.33%,16.96%,13.07%;Through factor analysis,it is concluded that the location of the disease is liver, spleen and kidney, followed by heart and gallbladder;the pathogenic factors are phlegm, blood stasis, dampness, heat,Qi deficiency, blood deficiency,Yin deficiency and Yang deficiency.TCM syndrome types are mainly divided into phlegm and blood stasis syndrome,Yang deficiency and blood stasis syndrome,Yin deficiency and blood stasis syndrome, dampness and heat stagnation syndrome,Qi deficiency and blood stasis syndrome, blood deficiency and blood stasis syndrome.Through Kmeans cluster analysis, numbness was divided into Yin deficiency and collateral stasis syndrome(32.86%),phlegm and blood stasis syndrome(25.09%),Qi deficiency and blood stasis syndrome(16.61%),blood deficiency and collateral stasis syndrome(12.72%),Yang deficiency and blood stasis syndrome(7.07%)and damp-heat obstruction syndrome(5.65%).In conclusion:Numbness(polyneuritis)is more common in middle-aged and elderly people, and the incidence of female is higher than that of male. Its common symptoms include needling sensation, ant movement sensation, hypoesthesia, pain, dry mouth, constipation and so on.The main tongue veins are dark red, with ecchymosis, thin white fur and fine veins. Its location is liver, spleen and kidney, followed by heart and gallbladder;phlegm, blood stasis, dampness, heat and deficiency are the most common pathogenic factors. The syndromes of numbness(polyneuritis)can be divided into six types: Yin deficiency and blood stasis syndrome, phlegm and blood stasis blocking collateral syndrome,Qi deficiency and blood stasis syndrome, blood deficiency and collateral stasis syndrome,Yang deficiency and blood stasis syndrome,damp-heat block syndrome, etc.
