Objective:To understand the rate of rehospitalization and mortality in patients with chronic heart failure within 1 year after discharge,and to analyze the relationship between Yiyangyang,Huoxue Lishui intervention and related factors and their endpoints..Methods:It is a retrospective study.Selected the hospitalized patients with CHF n the Cardiovascular Department of Hospital and the intensive care unit of coronary heart disease as the research object,from January 2014 to January 2018.For patients meeting the inclusion criteria,collect the general and clinical data,record the use of Chinese medicine.Besides,follow through calls and return visit,to reach the outcome or to one year,follow-up data included rehospitalization and survival,use of Western medicine outside the hospital,etc.Analyze collected data using spss20.0software.Results:A total of 163 patients with heart failure were collected in this study.74 cases(45.4%)of male,89cases(54.6%)of female,mean age 77.01±9.52 years old,83 patients(50.9%)in the 75-84 age group,33 patients(19%)aged?85 years old.NYHA class ? in97 cases(59.5%),Left ventricular ejection fraction?50at admission was 103 cases(63.2%).The cause is mainly 129 cases of coronary heart disease(78.5%),high heart disease was 14 cases(8.6%),dilated heart disease was 7cases(4.3%),others were 13cases(8.0%).CHF with disease:high blood pressure was 121 cases(74.2%)?lung infection was 85 cases(52.1%)?Arrhythmia was 73 cases(44.8%)?diabetes was 64 cases(39.3%)?cerebrovascular disease was 58 cases(35.6%)?old myocardial infarction was 49 cases(30.0%)?Average hospital stay 18.8±7.18 days,Taking Chinese medicine was 76 cases(46.6%),re-hospitalization rate was 45.4%within 1 year,Chinese medicine was 27 cases,1-year all-cause mortality rate is 9.2%?Univariate analysis showed statistically significant differences,include age,duration,NYHA functional class,taking the medicine,the hospital outside the medicine administration,the hospital referral cases(X2=8.257,P=0.039;X2=10.574,P=0.005;X2=7.778,P=0.02;X2=5.559,P=0.019;X2=6.634,P=0.011;X2=5.480,P=0.022)?Binary logistic regression analysis shows,Patients with CHF increased with age,and the risk of rehospitalization increased by 1.076times per year(OR=1.077,P=0.001).Compared with those who did not take Chinese medicine,the risk of rehospitalization in patients with chronic heart failure decreased by 2.253 times within 1 year(OR= 2.253,P=0.041).As the heart function gradually deteriorated from grade ? to grade ?,the risk of rehospitalization in patients with chronic heart failure increased by 3.398times(OR=0.940,P=0.001)within 1 year;compared with irregular follow-up,patients with chronic heart failure were re-hospital within 1 year.The risk is reduced by 3.279 times(OR=3.279,P=0.014)?Conclusion:Patients with CHF are mostly elderly,and there are many female patients.The cause is coronary heart disease,followed by hypertension,and hypertension is the most common disease.The proportion of Chinese medicine used during hospitalization still needs to be improved.With the combination of Chinese and Western medicine,the mortality rate has a downward trend.Univariate analysis showed that the factors associated with rehospitalization in patients with CHF were age,duration of disease,NYHA cardiac function grading,taking Chinese medicine,taking western medicine,and returning to the hospital.Binary logistic regression analysis showed that age,NYHA cardiac function grading,taking Chinese medicine,and outpatient review were related to the rate of rehospitalization of CHF.The use of traditional Chinese medicine during hospitalization can reduce the rate of rehospitalization in patients with chronic heart failure,but the rate of rehospitalization is still at a high level.For patients with severe heart failure,the combination of Chinese and Western medicine has a decreasing mortality rate.
