Clinical Study On The Treatment Of Gastroesophageal Reflux Disease With "Tiao Shen Yi Stomach" Acupuncture Method

Objective:Gastroesophageal reflux disease(GERD)is closely related to the psychological status of patients.In this study,three therapeutic methods,namely "regulating mind and benefiting stomach" acupuncture,conventional acupuncture and Western medicine therapy,were used to treat patients with GERD.The clinical effect and functional modification of"regulating mind and benefiting stomach" acupuncture on GERD at acupoints oftranquilizing mind and fixing mind based on the routine selection of acupoints in GERD were discussed from the change of the score index of Reflux Diagnostic Questionnaire(RDQ).Comparing with the common treatment methods at present,the therapeutic value of good is clarified in order to provide a safe and effective treatment for GERD.Methods:From May 2017 to December 2018,all the selected cases were from theDepartment of Spleen and Stomach Diseases and Rehabilitation Department of the Third Affiliated Hospital of Beijing University of Traditional Chinese Medicine.Ninety cases met the inclusion criteria and were selected as the subjects of study.According to the basic principles of randomized clinical research,the patients were divided into three groups by random number table method.Omeprazole control group,routine acupuncture group and"mind regulation to benefiting the stomach" acupuncture group,30 people in each group.Western medicine control group took Omeprazole 20mg once per day for 4 weeks as a course of treatment for 8 weeks.The routine acupuncture control group selected Zusanli,Neiguan,Zhongwan,Shanzhong,Tianshu and Taichong acupoints with the highest frequency of use according to the "consensus opinion of experts in TCM diagnosis and treatment of gastroesophageal reflux disease(2017)",which were treated once every 2 days,for a course of 4 weeks,for a total of 8 weeks.On the basis of routine acupoints(Shuangzusanli,Neiguan,Zhongwan,Shanzhong,Tianshu and Taichong),the "mind regulation to benefiting the stomach"acupuncture group added the acupoints of calming mind and mind:Yintang,Baihui and Anmian.Every 2 days,4 weeks as a course of treatment,a total of 8 weeks.RDQ was used to observe the four symptoms of heartburn,post-sternal pain,acid reflux and food reflux in the three groups before treatment,4 weeks after treatment and 8 weeks after treatment.Result:1)Evaluation of curative effect of RDQ after treatment in three groups:before treatment,4 weeks and 8 weeks after treatment in three groups,26 patients in the western medicine control group with total effective rate was 66.6%.In the conventional acupuncture group the total effective rate of this treatment was 72.2%.In the group of "regulating mind and benefiting stomach",27 patients recovered,the total effective rate was 79.3%.Statistical comparison showed that there was no significant difference in the total effective rate between the conventional acupuncture group and the western medicine control group(P>0.05),while the effect of the"mind regulation to benefiting the stomach"group was more significant than that of the former two groups(P<0.01).2)Evaluating the efficacy of RDQ scale on symptoms after 4 weeks of treatment:There was no significant difference in symptoms scores of heartburn,post-sternal pain,acid reflux and food reflux among the three groups before treatment(P>0.05).After 4 weeks of treatment,heartburn and acid regurgitation were significantly decreased in Western medicine group,routine acupuncture group and "mind regulation to benefiting the stomach" group(P<0.01),and the scores of post-sternum pain were also decreased in these three groups(P<0.05).There was no significant difference in the relief of heartburn,post-sternal pain and acid regurgitation among the three groups after 4 weeks of treatment(P>0.05).However,compared with western medicine group and conventional acupuncture group(P<0.05),the therapeutic effect of "mind regulation to benefiting the stomach" group was better than the former two groups(P<0.01).3)After 8 weeks of treatment,the symptoms of RDQ scale were evaluated.After 8 weeks of treatment,the effects of Western medicine control group,routine acupuncture group a nd"mind regulation to benefiting the stomach" group on heartburn,acid reflux and post-sternal pain were significantly reduced(P<0.01).Compared with western medicine group and routine acupuncture group,the effect of "mind regulation to benefiting the stomach"group on reflux was still better than the former two groups(P<0.05).4)The three groups of treatment methods can improve the clinical manifestations of heartburn,post-sternal pain and food reflux,and the "mind regulation to benefiting the stomach" acupuncture method is earlier than western medicine and conventional acupuncture method in improving post-sternal pain and acid reflux(after 4 weeks of treatment).5)One month later,81 patients were followed up by telephone in three groups.According to RDQ scale,the recurrence status of patients was evaluated.The recurrence rate of the groups was 29.2%for the omeprazole group,18.5%in routine acupuncture group;and 12.5%in the"mind regulation to benefiting the stomach"group.Conclusion:1)Mind regulation to benefiting the stomach Acupuncture can improve GERD symptoms by regulating brain-gut peptide hormones and related neurotransmitters of intestinal nervous system.2)The method of "mind regulation to benefiting the stomach" can effectively improve the heartburn of GERD patients.
