Purpose:To observe the effect that Shenfukang VI decoction combined with prednisone on rats adriamycin nephropathy model and glomerular electrostatic barrier,to investigate the treatment mechanism of Shenfukang VI in the adriamycin nephropathy model in rats.To provide a new train of thought for treating the small lesions of kidney disease progress and provide theoretical basis for the clinical application of Shenfukang VI.Material and method:Sixty male SD rats were randomly divided into normal group,model group,Chinese medicine group,prednisone group and combination group.There are 12 rats in each group.Except the normal group,adriamycin was injected twice every two weeks into the tail vein to establish adriamycin nephropathy model,and then every group was given corresponding drugs for four weeks.Observe the general state of rats and the 24hour's urine protein quantitation of each group every week after given drugs and before given drugs.After the experiment,the rats were taken blood to test the total protein,albumin,creatinine,urea,triglyceride and cholesterol.To observe the pathological changes of renal tissue,and the expression levels of PCX,HSPG and HPA by immunohistochemistry.Results:1.Model identification:Compared with the normal group(45.9±17mg/d),the 24-hour's urine protein quantitation was increased in the model group(135.4±7mg/d)(P<0.05).Compared with the model group,the 24hour's urine protein quantitation was decreased in the drug intervention group(P<0.05).It is successful for building model.2.General condition:After building groups by doxorubicin,every group appears these phenomenon that the activity decreases,hair messy,gloss reduction,spirit bad,low reaction,decrease into appetite,weight loss or no growth,loose defcate,urine volume reduction,some rats appear scrotal edema and ascites except normal group.The general condition of each drug intervention group was better than that of the model group.The amount of urine increased in the prednisone group and the combined group compared with the model group in the four week.The weight of traditional Chinese medicine group and combination group was increased,but that of prednisone group was not increased.3.Changes of 24 h urine protein quantitative in each: After the treatment,24 h urine protein quantitative was progressive increasing in each group.The 24hour's urine protein quantitation was decreased in the drug intervention group(P<0.05).The 24hour's urine protein quantitation of Chinese medicine group was significantly higher than prednisone group in the forth weeks.The 24hour's urine protein quantitation of combination group was decrease than the same period of prednisone group in the twice,third and forth week.4.Changes of serum biochemical indexes in each group: In the aspect of serum total protein and serum albumin,the drug intervention groups were slightly elevated than the model group,but the difference was not significant.There was no statistical difference between the groups in serum globulin.The serum triglyceride of normal group was lower than the other groups(P< 0.05),and the drug intervention group was lower than the model group(P<0.05),and there was no statistical difference between the treatment groups and the prednisone group(P>0.05).The serum cholesterol of every group was higher than normal group(P < 0.05)except normal group,and there was no statistical difference with the model group,and there was no statistical difference between the treatment groups and the prednisone group(P>0.05).There was no statistical difference between every group in erum creatinine and urea nitrogen(P>0.05).5.The renal tissue pathological changes: mild hyperplasia of mesangial cells and matrix in the model group,the drug intervention group have no obvious mesangial cells and matrix hyperplasia.The renal tubular protein pipe type and degree of renal interstitial edema of TCM group was decreased than the model group,prednisone group was decreased than the model group,and the combination group was decreased than the prednisone group.6.The level express of PCX,HSPG and HPA in each kidney by immunohistochemical: kidney PCX average optical density of each drug intervention group were higher than the model group(0.18 + 0.04)(P < 0.05),Chinese medicine group(0.23 + 0.34)was lower than the prednisone group(0.29 + 0.05)(P < 0.05),there was no statistical difference between the prednisone group and combination group(0.32 + 0.58)(P > 0.05);HPA average optical density of all the drug intervention group were lower than the model group(0.50 + 0.14)(P <0.05),the combination group(0.21 + 0.01)were lower than the prednisone group(0.29 + 0.09)(P < 0.05),there was no statistical difference between the Chinese medicine group(0.34-0.09)and the prednisone group(P > 0.05).There was no statistical difference between every group in average optical density values of the HSPG(P>0.05).Conclusion:1.The rat model of adriamycin was successfully replicated in this experiment.2.Shenfukang VI decoction can improve the general condition of rats,reduce 24 h urinary protein quantitation and serum triglycerides.But there was no statistical difference in serum total protein,serum albumin,serum cholesterol.Shenfukang VI decoction can improve the degree of kidney disease,and it is better to combine theprednisone.3.This mechanism may be related to lowering the expression level of HPA and raising the expression level of PCX protein and improve the glomerular electrical barrier.
