A Clinical Study On Xiaoya Kui Dekang Combined With Quadruple Therapy In The Treatment Of Gastric Ulcer With Accumulation Of Heat And Toxins

Purpose:To treat gastric ulcer of toxic heat accumulation type by combining the XiaoYongKuiDeKang decoction and the quadruple therapy,record and evaluate the improvement of TCM symptom score of patients and observe the healing of ulcer surfaces under gastroscope to provide a clinical basis for treating gastric ulcer of toxic heat accumulation type by combining traditional Chinese and Western medicine.Materials and Methods: Fifty-two patients who went to the clinic of the gastroenterology department of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from July 2016 to December 2018 and were diagnosed with gastric ulcer of toxic-heat accumulation type were selected for this clinical observation.Fifty-two patients were randomly divided into the test group and the control group,the test group has 27 patients and the control group has 25 patients.Patients of the control group was treated with the standard bismuth quadruple therapy:20 mg omeprazole enteric-coated capsule,oral administration,twice a day;220 mg bismuth potassium citrate capsule,oral administration,twice a day;1.0 g amoxicillin capsule,oral administration,twice a day;500 mg clarithromycin capsule,oral administration,twice a day.After 14 days of treatment,these patients were administrated with only 20 mg omeprazole enteric-coated capsule for 14 days(twice a day).While the patients of the test group were treated by combing the standard bismuth quadruple therapy and 150 ml XiaoYongKuiDeKang decoction(main ingredients include mongolian milkvetch root,coptidis rhizome,taraxacum,bletilla,etc.)twice a day.After 14 days of treatment,these patients were administrated with only 20 mg omeprazole enteric-coated capsule and 150 ml XiaoYongKuiDeKang decoction for 14 days(twice a day).The TCM symptom scores of patients before and after treatment,gastroscopy results and 13 C breath test results are recorded,and the total effective rates of both groups are analyzed and compared.Results:1.Comparison of symptom scores of patients: In the test group,clinical symptoms of10 patients were cured,clinical symptoms of 10 patients were significantly improved,clinical symptoms of 6 patients were improved,clinical symptoms of 1 patient were not improved,and the total effective rate was 96.29%.In the control group,clinical symptoms of 5 patients were cured,clinical symptoms of 6 patients were significantly improved,clinical symptoms of 9 patients were improved,clinical symptoms of 5patients were not improved,and the total clinical effective rate was 80%.The results showed that the total clinical effective rate of the test group was significantly higher than that of the control group,and the difference between both groups is statistically significant(P < 0.05).2.Comparison of effects on patients under gastroscopy: In the test group,clinical symptoms of 10 patients were cured,clinical symptoms of 9 patients were significantly improved,clinical symptoms of 6 patients were improved,clinical symptoms of 2 patients were not improved,and the total effective rate was 92.59%.In the control group,clinical symptoms of 5 patients were cured,clinical symptoms of 5patients were significantly improved,clinical symptoms of 9 patients were improved,clinical symptoms of 6 patients were not improved,and the total clinical effective rate was 76%.The total clinical effective rate of the test group was significantly higher than that of the control group,and the difference between both groups is statistically significant(P < 0.05).3.Comparison of Hp eradication rate: Both groups attended a 13 C breath test again 4weeks after treatment.In the test group,3 patients were Hp positive,24 patients were Hp negative,and the total eradication rate was 88.89%.In the control group,5patients were Hp positive,20 patients were Hp negative,and the total eradication rate was 80%.The Hp eradication rate of the test group was higher than that of the control group,and the difference between both groups is not statistically significant(P >0.05).Conclusion:1.The improvement of the clinical symptom scores and the ulcer healing of the test group were significantly higher than those of the control group.It showed that thecombination of the XiaoYongKuiDeKang decoction and the quadruple therapy was effective in treating of gastric ulcer of toxic heat accumulation type,providing a basis for treating gastric ulcer by combining traditional Chinese and Western medicine.2.The Hp eradication rate of the test group was higher than that of the control group,but there was no significant difference between both groups after statistical analysis.
