Objective:This study is to understand the current status of the skin around the intestinal stoma in patients with colorectal cancer,and to explore the related factors that affect the skin around the stoma in patients with colorectal cancer.To provide more effective clinical nursing interventions,improve the quality of life of colorectal cancer stoma patients and provide a reference for improving treatment outcomes.Material and method:This study is a cross-sectional study,using convenient sampling method,randomly selected 198 patients with colorectal cancer stoma from May 2019 to December 2019 in a third grade a cancer hospital in Liaoning Province for questionnaire survey.The research tools include general information questionnaire,skin assessment tool around the stoma,quality of life scale for patients with stoma,nutrition risk screening tool and self-management questionnaire for patients with enterostomy.All the data were collected by Excel and analyzed by SPSS 23.0.The general data and disease-related data of patients with stoma were described by frequency and percentage,and the measurement data were described by mean± standard deviation.Single factor analysis(Kruskal Wallis h test,Ridit analysis)and multi factor analysis(ordinal logistic regression)were used to analyze the related factors of skin injury around colostomy in patients with colorectal cancer.Results:1.Skin condition around the stoma of colorectal cancer patients: The evaluation score of the skin around the stoma is(4.77 ± 1.51),which is in a moderately damaged state.2.Single factor analysis of skin conditions around colorectal cancer stoma patients:gender,age,marital status,BMI,stoma location,stoma location,diabetes,radiotherapy and other 18 factors are different,the differences are statistically significant(P <0.05).3.Correlation analysis of the skin condition around the patient's enterostomy:(1)The total score of quality of life was negatively correlated with the total score of skin conditions around the enterostomy(r =-0.122,P <0.05).(2)The total score of nutritional risk is positively correlated with the total score of skin condition around the enterostomy(r = 0.203,P <0.01).(3)The total score of self-management is negatively correlated with the total score of skin conditions around the enterostomy(r =-0.155,P <0.05)4.The orderly Logistic regression results of the skin condition around the enterostomy in patients with colorectal cancer show that 9 variables such as age,diabetes,BMI,radiotherapy,psychological status,ostomy information management,psychological acceptance,nutritional status,and age score are entered Regression equations(b = 0.053,b = 2.260,b = 3.403,b =3.126,b = 0.062,b =-0.065,b =-0.359,b = 0.780,b = 0.284,P <0.05).Conclusion:Skin condition around the stoma in patients with colorectal cancer: more than four-fifths of patients have varying degrees of skin damage.Nearly two-thirds of patients had moderate and severe skin damage around the stoma,and about a quarter of patients had mild skin damage around the stoma.The older the age,the worse the psychological status,the poorer the ability to receive stoma information,the poorer the nutrition,the patients with diabetes,receiving radiation therapy,and the BMI value is too high or too low are more likely to cause damage to the skin around the stoma.
