| BackgroundCommunity-acquired pneumonia is a common clinical infectious disease.With the gradual aggravation of bacterial resistance,it brings difficulties to the anti-infection treatment of western medicine.Because of the multi-target and multi-link role of traditional Chinese medicine in the treatment of infectious diseases,the use of traditional Chinese medicine in the treatment of infectious diseases has attracted more and more attention from experts and scholars,and the effect of the dose of traditional Chinese medicine on clinical efficacy is an important topic.The basic research on the relationship between the quantity and effect of traditional Chinese medicine has made some progress,and it also faces many challenges.At present,the clinical research on the quantitative effect of Chinese medicine has gradually attracted the attention of experts.ObjectiveThis project based on the "The exterior-interior relationship between the lung and the large intestine" theory,intends to investigate the effect of Maxing Shigan decoction and Gegen Qinlian decoction combined with western medicine in the treatment of community-acquired pneumonia by observing the following problems:1.The clinical effect of Maxing Shigan decoction and Gegen Qinlian decoction combined with conventional western medicine in the treatment of community-acquired inflammation;2.Based on the conventional treatment of western medicine,the effect of Maxing Shigan decoction and Gegen Qinlian decoction on community acquired pneumonia in different dosage groups was observed,and the quantitative effect relationship of traditional Chinese medicine was preliminarily discussed.Method1.Ninety patients who met the inclusion and exclusion criteria were selected from the outpatient department or inpatient department of respiratory department of shunyi hospital of Beijing traditional Chinese medicine from May 2017 to December 2017.2.The patients were randomly divided into three groups(30 people in each group)using the random number table method,which were divided into the control group(conventional western medicine treatment group),small dose group(conventional western medicine treatment and small traditional Chinese medicine dose group),and large dose group(conventional western medicine treatment and large Chinese medicine dose group).(1)The control group was given routine antibiotics,expectorant,cough medicine,etc.(2)Small dose group was given Maxing Shigan decoction combined with Gegen Qinlian decoctionon the premise of western medicine treatment group,the composition of drugs and dosage is as follows: MaHuang 6g,Xingren 6g,Shigao 20 g,Zhigancao 5g,Gegen 15 g,Huangqin 9g,Huanglian 6g.Take one bag at a time,two times a day.(3)High-dose group was given Maxing Shigan decoction combined with Gegen Qinlian decoction according the soupon the premise of western medicine treatment group,the composition of drugs and dosage is as follows: MaHuang 12 g,Xingren 12 g,Shigao 40 g,Zhigancao 10 g,Gegen 30 g,Huangqin 18 g,Huanglian 12 g.Take one bag at a time,four times a day.All three groups of patients were treated for 7 days.3.The clinical symptoms before and after treatment of each group of patients and the change of inflammatory indicators(symptoms include cough,cough,chest pain,body temperature,thirst,sweat and other symptoms.Inflammation indicators include: WBC,NUET %,h-CRP)were observed,and related scoring tables were filled on the first,fourth,and seventh day of treatment。4.All data in this study were statistically processed by using SPSS 20.0.Results1.Effect on Community-acquired Pneumonia(1)In the control group,the patients were treated with Western medicine for routine antiinfection,cough,phlegm,and symptomatic treatment.On the fourth day,the patient’s cough,thirst,body temperature,and inflammatory indicators(WBC,N %,CRP)were improved compared with those before treatment and were statistically significant.(P<0.05);On the 7th day of treatment,cough,cough,sputum,chest pain,body temperature,thirst,and sweat improved significantly compared to the previous stage,and the inflammatory indicators(WBC,N %,CRP)decreased significantly and had statistical significance(P<0.05),indicating that Western medicine conventional treatment programs are effective.(2)On the fourth day of treatment in the small dose group,cough,chest pain,body temperature,thirst,and perspiration index(WBC,N %,CRP)all decreased compared to the previous day,and the symptoms improved more than before,which was statistically significant(P<0.05);On the 7th day of treatment,cough,chest pain,body temperature,thirst,and perspiration indicators(WBC,N %,CRP)were all improved compared to the pre-treatment and the 4th day of treatment,with statistical intent(P<0.05).(3)On the fourth day of treatment in the high-dose group,the patient’s cough,cough,chest pain,body temperature,thirst,and perspiration index(WBC,N %,CRP)were all lower than before,which was statistically significant(P<0.05);On the 7th day of treatment in the high-dose group,the patient’s cough,phlegm,chest pain,body temperature,thirst,and perspiration indicators(WBC,N %,CRP)were all lower than those before treatment and on the 4th day of treatment.Statistical significance(P<0.05).(4)Compared with the fourth day of the three groups of patients,the degree of cough,improved thirst,and percentage of neutrophils decreased in the small dose group,which was significantly different from the control group and had statistical significance(P<0.05);In the high dose group,cough,cough,chest pain,thirst,fever,neutrophils and CRP decreased significantly compared with the control group,and the results were statistically significant(P<0.05).(5)In each group of patients on the 7th day of treatment,the condition was stable,and the inflammation index and fever were normal.The small dose group had significant changes in thirst and sweat in the control group and had statistical significance(P<0.05).The patients with high dose were more effective in improving cough,cough,sputum,thirst,perspiration and CRP in the control group(P< 0.05).2.The relationship between the quantity and effect of traditional Chinese medicine:(1)On the fourth day of treatment,the treatment of cough,cough,phlegm,and chest pain symptoms in the high-dose group improved significantly,and the decline of C-reactive protein showed a significant trend and was statistically significant(P< 0.05).(2)On the seventh day of treatment,cough and phlegm in large dose group improved significantly in small dose group,and the results were statistically significant(P<0.05)Conclusion1.Maxing Shigan decoction a and Gegen Qinlian decoction combined with conventional western medicine is more effective than conventional western medicine in the treatment of community-acquired pneumonia.2.On the basis of conventional treatment of western medicine,Maxing Shigan decoction and Gegen Qinlian decoction had obvious therapeutic effect in the large dose group and the small dose group. |
