Analyze Professor Wang Sumei's Medication Experience In Treating Children With Tic Disorders Based On The Auxiliary Platform Of Chinese Medicine Inheritance

Objective:Using the Traditional Chinese Medicine Inheritance Support System(V2.5)to analyze the prescriptions of Professor Wang Sumei in the treatment of Tic Disorders(TD)in children,and summarize the medicine administration experience and academic thought given by Professor Wang Sumei for the treatment of children with TD.Methods:We collected and screened the cases of Professor Wang Sumei treating the children with TD in the pediatric clinic of The Oriental Hospital of Beijing University of TCM from January 2019 to December 2019.We Inputed them into the Traditional Chinese Medicine Inheritance Support System(V2.5)and established medical record database.Using frequency analysis,association rule analysis,complex system entropy clustering and other data mining methods,we analyzed the basic informations included the four gases,five flavors,channel tropism,category of medicines and frequency of medicines,and tried to find the medicine association rules and prescription rules,and we analyzed the core prescription and common drug combination,and extracted the new prescriptions.We combined with clinical practice and summed up Professor Wang Sumei's experience in the treatment of TD.Results:209 cases and 1386 prescriptions were collected in all,involving 230 traditional Chinese medicines.The main medicine properties were warm,cold and peaceful.The medicine tastes of the sweet,pungent used mostly,the bitter followed.The channel-tropism of medicine was mostly liver,and spleen,lung,stomach followed.111 medicines were used more than 30 times.The top 10 medicines with a frequency of more than 50%were Fangfeng,Gegen,Jiangcan,Gouteng,Shichangpu,Shenjincao,Quanxie,Gujingcao,Dilong,Tianma.The medicine categories of top 5 were relieving medicine,medicine for calming the liver and calming the wind,tonic medicine,antipyretic and antirheumatic medicine.Through the analysis of association rules,it was found that the commonly used drugs are Gegen and Fangfeng,Gegen and Jiangcan,Gegen and Shenjincao,Fangfeng and Jiangcan,Gegen and Gouteng,Fangfeng and Shenjincao,Fangfeng and Gouteng,Gegen,Fangfeng and Jiangcan,Fangfeng and Shichangpu,etc.;and the core drugs are Gegen,Fangfeng,Jiangcan,Quanxie,Gouteng,Shenjincao,Chenpi,Dilong,Tianma,Guizhi,Shichangpu,Gujingcao.By using complex system entropy clustering method,32 core combinations and 16 new prescriptions were acquired.New prescription 1 was from Sijunzi Decoction,which was suitable for children with TD with the symptoms of spleen deficiency,and it embodied the method of supporting soil with replenishing qi to invigorate the spleen.New prescription 2 was composed of Erchen Decoction and Liujunzi Decoction,which was suitable for convulsion with spleen deficiency and phlegm dampness blocking collaterals,and it embodied the method of supporting soil with strengthening the spleen and reducing phlegm.New prescription 3 was applicable for the heat treatment of liver of eye twitch,such as blink,rolling eyes or wink.New prescription 6 was cut from Tianma and Gouteng Decoction,which was applicable for the syndrome of hyperactivity of liver and wind,and embodied the method of suppressing wood with calming the liver.New prescription 7 can be used for the treatment of TD in children of Qi-Yin deficiency.New prescription 8 was used to treat the vocal twitch such as throat clearing and voice uttering.New prescriptions 9 and 10 were available for treating the nasal twitch.New prescriptions 3,8,9 and 10 embodied the treatment method of Professor Wang that combined syndrome differentiation and TCM syndrome differentiation.New prescription 11 can be used to treat the children of TD with enuresis due to liver and kidney loss.New prescription 12 was applicable to twitch children which had ADHA or sleep disorders with emotional disorder,inattention or restlessness of sleeping at night,and embodied the idea of regulating mind to restrain liver wood.New prescription 13 was a drug combination of Qi regulating and diet reducing,which was applicable to treat twitch children with abdominal distention,abdominal pain or poor appetite due to Qi stagnation,and embodied the method of suppressing wood with soothing the liver.New prescription 14 can be applied to treat TD in children of the loss of spleen and stomach and much sputum.New prescription 15 was used to treat twitch children accompanied by stagnation,which embodied the method of supporting soil with moving the spleen and eliminating stagnation.New prescription 16 can be used to treat twitch children which was accompanied by the syndrome of liver fire,and embodied the method of suppressing wood with clearing away heat and regulating the liver.Conclusions:Professor Wang Sumei treats TD based on the liver and spleen.She thinks that the basic pathogenesis of TD is spleen deficiency and hyperactivity of liver,and disturbance of wind and phlegm.The disease location is liver and spleen,and the disease nature is mixed with deficiency and excess.The treatment is based on the liver and spleen.And the core treatment method is rotary wood curbing soil.The methods of supporting soil mainly include strengthening the spleen and reducing phlegm,replenishing qi to invigorate the spleen,invigorating the spleen and eliminating stagnation.The methods of suppressing wood mainly consist of soothing the liver,invigorating the Yang and smoothing the liver,clearing away heat and regulating the liver,strengthening the body and regulating the liver.Professor Wang pays attention to the treatment of both the etiology and syndrome,and the combination of syndrome differentiation and TCM syndrome differentiation,according to the theories of the five organs,the long illness and stasis.She increases and decreases drugs with symptoms differentiation flexibly when treating children with TD.Professor Wang pays attention to the combination of cold and warm,and the principle of treatment is peaceful.The Commonly used medicine flavors are the sweet,the pungent and the bitter.The theory of channel tropism and medicinal guide is skillfully applied to treat TD in children.There are many kinds of medicines,such as relieving medicine,medicine for calming the liver and calming the wind,tonic medicine,antipyretic medicine.Professor Wang is especially good at using all kinds of wind dispelling drugs,aiming at strengthening the spleen,calming the liver,and relieving the wind and spasm.
