| The Shi Jinmo School is an important branch of Yanjing Medicine.It combines the characteristics of the family,academic,and integrated Chinese and Western medicine.It has very outstanding characteristics of traditional Chinese medicine in Beijing.Shi Jinmo,Zhu Shenyu,Lyu Renhe,Zhao Jinxi,Wang Shidong have inherited academic thought,and have continued to develop,with a clear central academic thought and a talent chain with a clear inheritance trajectory.The Shi Jinmo school has accumulated very valuable experience in the diagnosis and treatment of diabetes and its complications,.By collating and excavating the medical records of the five doctors of the Shi Jinmo school,comparing their respective characteristics and condensing common rules,it is of great significance to explore the rules and clinical characteristics of the Shi Jinmo school to treat diabetes and its complications,.Objective:In this study,by collecting and collating the medical records of 5 medical doctors including Shi Jinmo,Professor Zhu Shenyu,Professor Lyu Renhe,Professor Zhao Jinxi,and Professor Wang Shidong,applying modem information technology and data mining methods,this paper summarized and analyzed 5 doctors,s clinical medication rules and experience about applying the herbs of promoting blood circulation and removing blood stasis to treat diabetes.Methods:According to the inclusion and exclusion criteria,a total of 1210 cases of diabetes were included in this study.The data of 1210 medical records(including disease diagnosis,information of four diagnoses,names of traditional Chinese medicine,etc.)were standardized,and the data were inputted and the database was established.The method of frequency statistics was used to count the basic information of medical records,disease diagnosis,four diagnostic information,efficacy classification,and the dosage of drugs for promoting blood circulation and removing blood stasis,such as four qi,five flavors,channel tropism,activating blood circulation and removing blood stasis.The compatibility of drugs for promoting blood circulation and removing blood stasis in five doctors were analyzed by association rules and cluster analysis,and the association rules between symptoms,drugs and drug efficacy were analyzed respectively.Chi-square test was used to analyze the differences in the application of single medicine for promoting blood circulation and removing blood stasis and the combination of drugs for promoting blood circulation and removing blood stasis in different stages and complications of diabetes.Results:The top 10 diagnoses were:type 2 diabetes,diabetic nephropathy,hypertension,hyperlipidemia,hyperuricemia,diabetic retinopathy,abnormal liver function,fatty liver,cerebral infarction,coronary atherosclerotic heart disease.The top 10 symptoms in medical records were fatigue,insomnia,thirst,edema,blurred vision,lumbar acid,dry stool,nocturnal urination,foam urine and numbness.The top 10 tongue images in medical records are as follows:red tongue,dark tongue,pale tongue,white fur,thin fur,greasy fur,yellow fur,tooth marks on the edge,fat tongue and red tip of tongue.The top 10 pulse appearance frequency in medical records are:thin pulse,deep pulse,pulse string,pulse slippery,pulse number.The efficacy of the top three drugs in the frequency of the prescription are deficiency medicine,heat-clearing drug,blood-activating and stasis-removing medicine in turn.In the prescriptions of Mr.Shi Jinmo,the frequency of drugs for tonifying deficiency and clearing heat was significantly higher than that of promoting blood circulation and removing blood stasis,while there was little difference in the frequency of these three kinds of drugs in the prescriptions of the other four professors.According to the order of support from high to low,Professor Zhu Shenyu,Professor Lyu Renhe,Professor Zhao Jinxi and Professor Wang Shidong all ranked in the forefront in the correlation intensity between promoting blood circulation and regulating menstruation and tonifying qi.In addition,Professor Zhu Shenyu and Professor Lyu Renhe’s prescriptions for promoting blood circulation and regulating menstruation and tonifying qi are also strongly related to drugs for clearing away heat and cooling blood,while Professor Zhao Jinxi’s prescriptions are strongly related to drugs for promoting blood circulation and regulating menstruation,tonifying qi and tonifying blood.In the prescriptions of Mr.Shi Jinmo and Professor Zhu Shenyu,the statistics of "four qi"of promoting blood circulation and removing blood stasis drugs were the most common,while those of Lyu Renhe,Zhao Jinxi and Wang Shidong were the most common in the statistics of "four qi" of promoting blood circulation and removing blood stasis in the prescriptions of professors Lyu Renhe,Zhao Jinxi and Wang Shidong.Among the prescriptions of the five physicians,the statistics of "five flavors" of promoting blood circulation and removing blood stasis are the most common,and the statistics of "meridian tropism" are the most common in the liver meridian.Professor Lyu Renhe’s prescription has the lowest average flavor number,which is only 11,and the prescription size is concentrated in 5 to 20 flavors,while Professor Zhao Jinxi’s prescription has the highest average flavor number,which is 19.4,and the prescription size varies from 8 to 33.There was no significant difference in the average taste number of the prescriptions of the remaining three professors.Mr.Shi Jinmo’s prescription was 15.9,Professor Zhu Shenyu’s prescription was 15.6,and Professor Wang Shidong’s prescription was 16.9.Dosage of drugs for promoting blood circulation and removing blood stasis:the average dose of drugs for promoting blood circulation and removing blood stasis in Professor Lyu Renhe’s prescription(16.9 g±9.3 g)and the average dose of activating blood circulation and removing blood stasis(42.48%±14.46%)were the largest.In Mr.Shi Jinmo’s prescription,the average dose of activating blood circulation and removing blood stasis(13.2g±9.8g)and the average dose of activating blood circulation and removing blood stasis(7.26%±4.95%)were the lowest.In Professor Zhu Shenyu’s prescription,the discretization degree of the dose of promoting blood circulation and removing blood stasis is the largest,and the dose range is relatively wide,with an average dose of 15.8g±11.0g.In Professor Zhao Jinxi’s prescription,the discretization degree of the dose of promoting blood circulation and removing blood stasis was the lowest,and the average dose was 15.6g±7.6g.The average dose of promoting blood circulation and removing blood stasis in Professor Wang Shidong’s prescription was 15.5g±8.0g.Common drug combinations include Angelica sinensis,Ligusticum chuanxiong,peach kernel,safflower,dog ridge,Dipsacus,Radix Astragali,Radix angelicae Sinensis,Bupleurum,Radix Paeoniae Rubra,Radix Salviae Miltiorrhizae,Pueraria lobata.Professor Chen Yu’s individual drug combinations are:Wulingzhi,Cyperus roxburghii,Mulberry parasite,Chicken Blood Rattan,Radix Paeoniae Rubra and Dilong.Professor Lyu Renhe’s individual drug combinations are:Salvia miltiorrhiza,peony bark,red peony,deer antlers,tortoise shell,Cyperus,black medicine,Rhodiola,Ganoderma lucidum.Professor Zhao Jinxi’s individual drug combinations are:rhubarb,cicada slough,turmeric,Bombyx mori,Radix Paeoniae Alba,Radix Paeoniae Rubra,pyrrosia lingua pyrrosia lingua,scallops,mountain dragons,ghost arrow feathers,burdock.Professor Wang Shidong’s individual drug combinations are Corydalis,toosendan,turmeric,Acorus tatarinowii,Cortex moutan,alisma,Cornus officinalis,rhubarb,Magnolia officinalis,Fructus AurantiiAnalysis of the application of drugs for promoting blood circulation and removing blood stasis by stages:a total of 13 drugs for promoting blood circulation and removing blood stasis and 7 groups of drugs for promoting blood circulation and removing blood stasis were significantly different in the period of diabetes and diabetes.In the cases of diabetes,the core drugs of promoting blood circulation and removing blood stasis in the prescriptions of professors Zhu Shenyu,Lyu Renhe,Zhao Jinxi and Wang Shidong were screened out respectively.The core drugs of promoting blood circulation and removing blood stasis in the prescriptions of professors Zhu Shenyu,Lyu Renhe,Zhao Jinxi and Wang Shidong were screened out in the cases of Xiaozhuo stage,6 flavor,5 flavor,7 flavor and 4 flavor in the prescriptions of professors respectivelyA total of 11 drugs for promoting blood circulation and removing blood stasis and the combination of two groups of drugs for promoting blood circulation and removing blood stasis were significantly different in diabetic nephropathy,diabetic retinopathy and diabetic peripheral neuropathy.Among the cases of diabetic nephropathy,5 and 9 flavors of core drugs for promoting blood circulation and removing blood stasis were selected by Professor Lyu Renhe and Zhao Jinxi respectively.In the cases of diabetic retinopathy,5 and 9 flavors of core drugs for promoting blood circulation and removing blood stasis were selected from the prescriptions of Lyu Renhe and Zhao Jinxi respectively.In the cases of diabetic peripheral neuropathy,6 and 9 kinds of core drugs for promoting blood circulation and removing blood stasis were selected from the prescriptions of Lyu Renhe and Zhao Jinxi respectively.Conclusion:Professors Zhu Shenyu,Lyu Renhe,Zhao Jinxi and Wang Shidong all paid attention to the application of drugs for promoting blood circulation and removing blood stasis in the treatment of diabetes and its complications,while the frequency of drugs for promoting blood circulation and removing blood stasis was lower in Mr.Shi Jinmo’s prescription.From the perspective of the application of drugs for promoting blood circulation and removing blood stasis in different stages of diabetes,Salvia miltiorrhiza and Radix Paeoniae Rubra are the common core drugs for promoting blood circulation and removing blood stasis in the prescriptions of Zhu Shenyu,Lyu Renhe,Zhao Jinxi and Wang Shidong.It is suggested that the four professors attach importance to the drugs of promoting blood circulation and removing blood stasis and clearing heat during diabetes and diabetes.From the perspective of the application of drugs for promoting blood circulation and removing blood stasis in different complications of diabetes,Salvia miltiorrhiza,Radix Paeoniae Rubra,Angelica sinensis and Ligusticum chuanxiong are the common core drugs for promoting blood circulation and removing blood stasis of Professor Lyu Renhe and Professor Zhao Jinxi.It is suggested that the two professors attach importance to the use of drugs for cooling blood and promoting blood circulation,replenishing qi and activating blood circulation,regulating qi and activating blood circulation in the treatment of three major complications.From the analysis of the taste of prescription drugs and the dose of drugs for promoting blood circulation and removing blood stasis,Professor Zhu Shenyu is the most flexible in adjusting the dosage of drugs for promoting blood circulation and removing blood stasis;Professor Lyu Renhe has a concise prescription and is good at using "drugs",and the dosage of drugs for promoting blood circulation and removing blood stasis is on the high side;while Professor Zhao Jinxi has rich prescriptions and is good at using "prescriptions",and the dosage of drugs for promoting blood circulation and removing blood stasis is more regular.From the point of view of the compatibility of drugs for promoting blood circulation and removing blood stasis,professors Zhu Shenyu,Lyu Renhe,Zhao Jinxi and Wang Shidong often used drugs for promoting blood circulation and removing blood stasis to be compatible with drugs for tonifying qi.Professor Zhu Shenyu and Professor Lyu Renhe often combine drugs for clearing heat and cooling blood on the basis of drugs for promoting blood circulation and removing blood stasis and tonifying qi,while Professor Zhao Jinxi often prescribes drugs for tonifying blood on the basis of drugs for promoting blood circulation and removing blood stasis and tonifying qi.In the combination of drugs for promoting blood circulation and removing blood stasis,there are not only common drug combinations,but also individual drug combinations. |
